围绝经期抑郁症患者血清E2、TSH、FT3、FT4水平与抑郁严重程度的关系OA
Relationship Between Serum E2,TSH,FT3,and FT4 Levels and Depression Severity in Patients with Perimenopausal Depression
目的 探讨围绝经期抑郁症患者血清雌二醇(E2)、促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)水平与抑郁严重程度及重度抑郁分组的相关性.方法 采用横断面观察性研究方法,连续选取 2021年 10 月至 2025 年 10 月郑州市第八人民医院收治的 125 例围绝经期抑郁症患者为研究对象,根据抑郁程度分为轻中度抑郁组(93 例)和重度抑郁组(32 例);比较两组一般临床资料,入院时血清 E2、TSH、FT3、FT4 水平,并分析血清E2、TSH、FT3、FT4 水平与汉密尔顿抑郁量表(HAMD)评分的相关性,采用 Logistic 回归分析血清 E2、TSH、FT3、FT4 水平与围绝经期抑郁症患者重度抑郁分组的统计学关联.结果 重度抑郁组改良Kupperman评分高于轻中度抑郁组,病程长于轻中度抑郁组,差异均有统计学意义(P<0.05);入院时重度抑郁组血清 E2、TSH、FT3、FT4 水平均低于轻中度抑郁组,差异均有统计学意义(P<0.05);血清 E2、TSH、FT3、FT4 水平与 HAMD 评分均呈负相关(r=-0.704、-0.581、-0.637、-0.519,P<0.05);Logistic 回归分析显示,血清 E2、TSH、FT3、FT4 水平降低与围绝经期抑郁症患者重度抑郁分组存在统计学关联(P<0.05).结论 血清 E2、TSH、FT3、FT4 水平降低与围绝经期抑郁症患者 HAMD评分升高及重度抑郁分组存在相关性,可能为抑郁严重程度评估提供一定参考.
Objective To investigate the correlations of serum estradiol(E2),thyroid-stimulating hormone(TSH),free triiodothyronine(FT3),and free thyroxine(FT4)levels with depression severity and severe depression grouping in patients with perimenopausal depression.Methods A cross-sectional observational study was conducted.A total of 125 patients with perimenopausal depression who were consecutively admitted to Zhengzhou Eighth People's Hospital from October 2021 to October 2025 were selected as the study subjects.According to the severity of depression,the patients were divided into the mild-to-moderate depression group(n=93)and the severe depression group(n=32).General clinical data and serum E2,TSH,FT3,and FT4 levels at admission were compared between the two groups.The correlations between serum E2,TSH,FT3,and FT4 levels and Hamilton Depression Rating Scale(HAMD)scores were analyzed.Logistic regression analysis was used to evaluate the statistical associations between serum E2,TSH,FT3,and FT4 levels and severe depression grouping in patients with perimenopausal depression.Results The modified Kupperman score was higher and the disease duration was longer in the severe depression group than in the mild-to-moderate depression group,and the differences were statistically significant(P<0.05).At admission,serum E2,TSH,FT3,and FT4 levels in the severe depression group were lower than those in the mild-to-moderate depression group,and the differences were statistically significant(P<0.05).Serum E2,TSH,FT3,and FT4 levels were negatively correlated with HAMD scores(r=-0.704,-0.581,-0.637,and-0.519,respectively;P<0.05).Logistic regression analysis showed that decreased serum E2,TSH,FT3,and FT4 levels were statistically associated with severe depression grouping in patients with perimenopausal depression(P<0.05).Conclusion Decreased serum E2,TSH,FT3,and FT4 levels are correlated with increased HAMD scores and severe depression grouping in patients with perimenopausal depression,and these indicators may provide a certain reference for assessing depression severity.
