目的:为解决甲状腺手术后嗓音恢复缺乏客观量化监测方法的问题,基于梅尔频率倒谱系数(Mel-frequency cepstral coefficient,MFCC)特征的动态时间规整(dynamic time warping,DTW)距离(MFCC-DTW 距离)定量评估甲状腺手术后嗓音的恢复情况.方法:采集54名甲状腺结节切除手术患者的术前1 d及术后12、24、48 h和6个月的声音信号,并同时记录总体嘶哑程度、粗糙声、气息声、无力声、紧张声(Grade,Roughness,Breathiness,Asthenia,Strain,GRBAS)评分.分析术前和术后声音信号的频谱特征和MFCC特征,并利用DTW算法计算手术前后MFCC特征的DTW距离,实现对发声系统恢复的监测评估.对比MFCC-DTW距离与GRBAS评分结果的一致性.结果:患者声音信号频谱特征和MFCC特征变化与患者恢复情况高度吻合.54名患者的术后12、24、48 h和6个月嗓音与术前嗓音的平均MFCC-DTW距离分别为246、204、190和85,呈现随恢复进程逐渐下降的趋势(P<0.05).术后12、24和48 h的MFCC-DTW距离与GRBAS评分的Spearman相关系数分别为0.908、0.903和0.910(P均<0.01).术后6个月,多数患者嗓音功能已基本恢复,GRBAS评分集中在0分或接近0分,MFCC-DTW距离与GRBAS评分的Spearman相关系数为0.642.结论:提出的方法可有效量化甲状腺术后嗓音的恢复进度,为甲状腺术后嗓音恢复的跟踪评估提供了更为便捷与可靠的客观依据.

Objective To quantitatively assess voice recovery following thyroid surgery with the Mel-frequency cepstral coefficient(MFCC)-based dynamic time warping(DTW)distance(MFCC-DTW distance),so as to solve the problems of postoperative voice recovery monitoring in lacking objective evaluation methods.Methods Voice signals were collected from 54 patients undergoing thyroid nodule resection 1 day before surgery and 12,24,48 hours and 6 months after surgery,and the scores for the Grade,Roughness,Breathiness,Asthenia and Strain(GRBAS)were recorded simultaneously.The spectral characteristics and MFCC features of preoperative and postoperative voice signals were analyzed.The DTW algorithm was then used to calculate the DTW distance between preoperative and postoperative MFCC features,thereby enabling monitoring and evaluation of vocal function recovery.The consistency between MFCC-DTW distance and GRBAS scores was further explored.Results Changes in the spectral characteristics and MFCC features of patients' voice signals were highly consistent with their recovery status.The mean MFCC-DTW distances between the postoperative voices of the 54 patients at 12,24,and 48 hours and at 6 months and their preoperative voices were 246,204,190 and 85,respectively,showing a gradual decrease as recovery progressed(P<0.05).The Spearman correlation coefficients between the MFCC-DTW distance and the GRBAS scores at 12,24 and 48 hours postoperatively were 0.908,0.903 and 0.910,respectively(all P<0.01).Most the patients regained largely their vocal function 6 months after surgery,with GRBAS scores concentrated at 0 or close to 0;the Spearman correlation coefficient between the MFCC-DTW distance and the GRBAS score was 0.642.Conclusion The proposed method effectively quantifies the recovery progress of voice function after thyroid surgery,and facilitates convenient and reliable follow-up evaluation of postoperative voice recovery.[Chinese Medical Equipment Journal,2026,47(6):20-28]
