基于MRI的影像组学特征联合NLR、SIRI对三阴性乳腺癌患者术后复发转移的预测价值OA
Predictive Value of MRI-Based Radiomics Features Combined with NLR and SIRI for Postoperative Recurrence and Metastasis in Patients with Triple-Negative Breast Cancer
目的 探讨基于磁共振成像(MRI)的影像组学特征联合中性粒细胞/淋巴细胞比值(NLR)、全身炎性反应指数(SIRI)对三阴性乳腺癌(TNBC)患者术后复发转移的预测价值.方法 回顾性选取2019年1月至2022年12月我院收治并经术后病理确诊的TNBC患者95例,所有患者术前均行乳腺MRI检查,筛选影像组学特征并计算影像组学评分,同时术前采集外周血检验结果计算NLR、SIRI.患者均完成术后3年随访,根据随访期内是否出现复发转移分为复发转移组与非复发转移组.比较两组患者的一般资料、疾病相关信息、MRI影像组学评分、NLR、SIRI,采用多因素Logistic回归分析筛选TNBC患者术后复发转移的独立危险因素,绘制受试者工作特征(ROC)曲线评估MRI影像组学评分、NLR、SIRI单独与三者联合对TNBC患者术后复发转移的预测效能.结果 随访3年,95例TNBC患者中有29例(30.53%)发生复发转移,66例(69.47%)未发生复发转移;复发转移组与非复发转移组的肿瘤大小、淋巴结状态、组织学分级、Ki-67指数、MRI影像组学评分、NLR、SIRI组间比较,差异具有统计学意义(P<0.05);多因素Logistic回归分析结果显示,高MRI影像组学评分、高NLR及高SIRI为TNBC患者术后复发转移的独立危险因素(P<0.05);MRI影像组学评分、NLR、SIRI三项指标联合预测TNBC患者术后复发转移的曲线下面积(AUC)、灵敏度、特异度均优于单一指标预测(P<0.05).结论 基于MRI的影像组学特征联合NLR、SIRI可有效预测TNBC患者术后复发转移风险,能为患者个体化分层随访与辅助治疗方案制定提供客观参考依据.
Objective To investigate the predictive value of magnetic resonance imaging(MRI)-based radiomics features combined with neutrophil-to-lymphocyte ratio(NLR)and systemic inflammation response index(SIRI)for postoperative recurrence and metastasis in patients with triple-negative breast cancer(TNBC).Methods A total of 95 patients pathologically diagnosed with triple-negative breast cancer(TNBC)after surgery admitted to our hospital from January 2019 to December 2022 were retrospectively enrolled.All patients underwent preoperative breast MRI examination;radiomics features were screened and radiomics scores were calculated.Meanwhile,preoperative peripheral blood test results were collected to calculate NLR and SIRI.All patients completed a 3-year postoperative follow-up and were divided into recurrence/metastasis group and non-recurrence/metastasis group based on whether recurrence or metastasis occurred during the follow-up period.General data,disease-related information,MRI radiomics scores,NLR,and SIRI levels were compared between the two groups.information,MRI radiomics scores,NLR,and SIRI were compared between the two groups.Multivariate Logistic regression analysis was used to identify independent risk factors for postoperative recurrence and metastasis in TNBC patients.Receiver operating characteristic(ROC)curves were drawn to evaluate the predictive efficacy of MRI radiomics score,NLR,SIRI alone and their combination for postoperative recurrence and metastasis in TNBC patients.Results During the 3-year follow-up,29 case of the 95 TNBC patients(30.53%)developed recurrence or metastasis,while 66 cases(69.47%)did not.There were statistically significant differences in tumor size,lymph node status,histological grade,Ki-67 index,MRI radiomics score,NLR,and SIRI between the recurrence/metastasis group and the non-recurrence/metastasis group(P<0.05).Multivariate Logistic regression analysis showed that high MRI radiomics score,high NLR,and high SIRI were independent risk factors for postoperative recurrence and metastasis in TNBC patients(P<0.05).The area under the curve(AUC),sensitivity,and specificity of the combined prediction using MRI radiomics score,NLR,and SIRI were all superior to those of any single indicator(P<0.05).Conclusions MRI-based radiomics features combined with NLR and SIRI can effectively predict the risk of postoperative recurrence and metastasis in TNBC patients,providing an objective reference for individualized stratified follow-up and adjuvant treatment decision-making.
