目的 探讨超声影像组学对ST段抬高型心肌梗死(STEMI)患者再灌注治疗后心肌损伤评估的价值.方法 回顾性分析2023年2月至2026年1月期间在本院接受经皮冠状动脉介入(PCI)治疗的100例STEMI患者的临床资料,依据是否存在微血管阻塞或心肌内出血将患者分为心肌损伤组52例与非心肌损伤组48例.PCI术后第4天完成经胸超声心动图检查,分析Rad评分及常规超声参数诊断再灌注后心肌损伤的价值.结果 心肌损伤组Rad评分、E/e'、左心房容积指数高于非心肌损伤组,LVEF、GLS、e'低于非心肌损伤组(P<0.05).多因素Logistic回归分析结果显示Rad评分、GLS降低及E/e'升高为STEMI再灌注后心肌损伤的独立危险因素(P<0.05).Rad评分、GLS、E/e'及联合模型诊断STEMI患者再灌注后心肌损伤的AUC分别为0.855、0.806、0.820、0.933,联合模型诊断心肌损伤的AUC高于Rad评分、GLS、E/e'(P<0.05).结论 Rad评分、GLS、E/e'与STEMI患者再灌注后心肌损伤关系密切,联合应用可提高对心肌损伤诊断价值.

Objective To investigate the value of ultrasound radiomics in evaluating myocardial injury after reperfusion therapy in patients with ST-segment elevation myocardial infarction(STEMI).Methods The clinical data of 100 STEMI patients who underwent percutaneous coronary intervention(PCI)at our hospital from February 2023 to January 2026 were retrospectively analyzed.According to the presence of microvascular obstruction or intramyocardial hemorrhage,the patients were divided into myocardial injury group(52 cases)and non-myocardial injury group(48 cases).Transthoracic echocardiography was performed on the 4th day after PCI,and the diagnostic value of the Rad-score and conventional echocardiographic parameters for myocardial injury after reperfusion was assessed.Results The Rad-score,E/e',and left atrial volume index in the myocardial injury group were higher than those in the non-myocardial injury group,while LVEF,GLS,and e'were lower than those in the non-myocardial injury group(all P<0.05).Multivariate Logistic regression analysis showed that Rad-score,decreased GLS,and elevated E/e'were independent risk factors for myocardial injury after STEMI reperfusion(P<0.05).The areas under the curve(AUC)of Rad-score,GLS,E/e',and the combined model for diagnosing myocardial injury after reperfusion in STEMI patients were 0.855,0.806,0.820,and 0.933,respectively.The AUC of the combined model was significantly higher than those of Rad-score,GLS,and E/e'alone(P<0.05).Conclusions Rad-score,GLS,and E/e'are closely associated with myocardial injury after reperfusion in STEMI patients,and their combined application can improve the diagnostic value.
