超声心动图联合组织多普勒成像评价心肌缺血患者心功能与左室收缩同步性的应用价值OA
Application Value of Echocardiography Combined with Tissue Doppler Imag-ing in Evaluating Cardiac Function and Left Ventricular Systolic Synchrony in Patients with Myocardial Ischemia
目的 探讨超声心动图联合组织多普勒成像(TDI)在心肌缺血患者心功能与左室收缩同步性评价中的应用价值.方法 选取2024年4月至2026年1月在赣州市南康区第二人民医院确诊的心肌缺血患者35例作为研究组,同期选取35例健康体检者作为对照组.两组均行常规超声心动图及TDI检查,检测并比较LVEF、LVEDD、LVESD等心功能指标,以及s'、e'、a'、e'/a'比值、Ts-SD等TDI参数;绘制ROC曲线,分析单一指标与联合指标对心肌缺血的诊断效能.结果 研究组的LVEF低于对照组,LVEDD、LVESD高于对照组(P<0.05).研究组的s'、e'、e'/a'低于对照组,a'、Ts-SD高于对照组(P<0.05).ROC曲线分析显示,超声心动图指标联合TDI参数诊断心肌缺血的AUC为0.959,灵敏度0.943,特异度0.943,约登指数0.886,诊断效能优于单一指标.结论 超声心动图与TDI可准确反映心肌缺血患者的心功能损害及左室收缩不同步,二者联合应用能提升心肌缺血的诊断准确性,可为临床诊疗提供客观依据.
Objective To investigate the application value of echocardiography combined with tissue Doppler imaging(TDI)in the evaluation of cardiac function and left ventricular systolic synchrony in patients with myocardial ischemia.Methods A total of 35 patients diagnosed with myocardial ischemia in Nankang District Second People's Hospital of Ganzhou City from April 2024 to January 2026 were selected as the study group,and 35 healthy controls during the same period were selected as the control group.Both groups underwent conventional echocardiography and TDI examinations.Cardiac function indicators including LVEF,LVEDD and LVESD,as well as TDI parameters including s',e',a',e'/a'ratio and Ts-SD were detected and compared between the two groups.ROC curve was drawn to analyze the diagnostic performance of single indicators and combined indicators for myocardial ischemia.Results The LVEF in the study group was lower than that in the control group,while LVEDD and LVESD were higher than those in the control group(P<0.05).The s',e'and e'/a'in the study group were lower than those in the control group,while a'and Ts-SD were higher than those in the control group(P<0.05).ROC curve analysis showed that the AUC of combined echocardiographic indicators and TDI parameters for the diagnosis of myocardial ischemia was 0.959,with a sensitivity of 0.943,a specificity of 0.943 and a Youden index of 0.886,and its diagnostic performance was superior to that of any single indicator.Conclusions Echocardiography and TDI can accurately reflect cardiac function impairment and left ventricular systolic dyssynchrony in patients with myocardial ischemia.The combined application of the two can improve the diagnostic accuracy of myocardial ischemia and provide an objective basis for clinical diagnosis and treatment.
