左侧背外侧前额叶皮层与左侧外侧颞叶皮层重复经颅磁刺激治疗老年痴呆的效果分析OA
Analysis on the Effect of Repetitive Transcranial Magnetic Stimulation of the Left Dorsolateral Prefrontal Cortex and Left Lateral Temporal Cortex in the Treatment of Senile Dementia
目的 探讨左侧背外侧前额叶皮层与左侧外侧颞叶皮层重复经颅磁刺激(rTMS)治疗老年痴呆的临床效果,为临床诊疗提供参考.方法 选取2023年9月至2025年9月期间衢州市第三医院收治的60例老年痴呆患者为研究对象,以随机数字表法分为对照组和观察组,各30例.两组患者均予以基础治疗,在此基础上,对照组患者行rTMS伪刺激治疗,观察组患者行左侧背外侧前额叶皮层与左侧外侧颞叶皮层rTMS治疗.两组患者均连续治疗8周.对比两组认知功能、事件相关电位、日常生活能力.结果 两组患者ADAS-cog、MMSE、ACE-Ⅲ、MBI评分及N1潜伏期、P2潜伏期、P3波幅均具有时间、组间、交互效应差异.治疗4周、8周后,两组ADAS-cog评分均呈下降趋势,且观察组均低于对照组(P<0.05);两组患者MMSE、ACE-Ⅲ、MBI评分均呈升高趋势,且观察组均高于对照组(P<0.05).治疗4周、8周后,两组患者N1潜伏期、P2潜伏期均呈缩短趋势,且观察组均短于对照组(P<0.05);两组患者P3波幅均呈上升趋势,且观察组均高于对照组(P<0.05).结论 左侧背外侧前额叶皮层与左侧外侧颞叶皮层rTMS应用在老年痴呆患者中,有利于提升其认知功能,改善事件相关电位,提高其日常生活能力.
Objective To investigate the clinical efficacy of repetitive transcranial magnetic stimulation(rTMS)of the left dorsolateral prefrontal cortex and left lateral temporal cortex in the treatment of senile dementia,and to provide reference for clinical diagnosis and treatment.Methods A total of 60 patients with senile dementia admitted to Quzhou Third Hospital from September 2023 to September 2025 were enrolled as the research subjects.They were divided into the control group and the observation group by the random number table method,with 30 cases in each group.Both groups received basic treatment.On this basis,the control group received sham rTMS treatment,while the observation group received rTMS treatment of the left dorsolateral prefrontal cortex and left lateral temporal cortex.Both groups were treated continuously for 8 weeks.The cognitive function,event-related potentials,and activities of daily living were compared between the two groups.Reslults There were significant time effects,group effects,and interaction effects on the scores of Alzheimer's Disease Assessment Scale-Cognitive Subscale(ADAS-cog),Mini-Mental State Examination(MMSE),Addenbrooke's Cognitive Examination-Ⅲ(ACE-Ⅲ),Modified Barthel Index(MBI),as well as N,latency,P2 latency,and P3 amplitude between the two groups(all P<0.05).After 4 and 8 weeks of treatment,the ADAS-cog scores of both groups showed a downward trend,and those in the observation group were significantly lower than those in the control group(P<0.05);the MMSE,ACE-Ⅲ,and MBI scores of both groups showed an upward trend,and those in the observation group were significantly higher than those in the control group(P<0.05).After 4 and 8 weeks of treatment,the N,latency and P2 latency of both groups showed a shortening trend,and those in the observation group were significantly shorter than those in the control group(P<0.05);the P3 amplitude of both groups showed an increasing trend,and that in the observation group was significantly higher than that in the control group(P<0.05).Conclusions rTMS of the left dorsolateral prefrontal cortex and left lateral temporal cortex is beneficial for improving cognitive function,event-related potentials,and activities of daily living in patients with senile dementia.
