MRI对颞下颌关节盘变性移位的诊断价值及影像学特征分析OA
Diagnostic Value of MRI for Temporomandibular Joint Disc Degeneration and Displacement and Analysis of Imaging Features
目的 探讨磁共振成像(MRI)对颞下颌关节盘变性移位的诊断价值,并分析其影像学特征.方法 选取2025年1月至2025年6月在兰州市第二人民医院就诊的67例疑似颞下颌关节盘变性移位患者作为研究对象,所有患者均接受MRI检查.以临床综合诊断结果作为金标准,评估MRI诊断颞下颌关节盘变性移位的效能,并比较颞下颌关节盘变性移位与非颞下颌关节盘变性移位患者的MRI征象差异.结果 67例患者经临床综合诊断确诊颞下颌关节盘变性移位62例,非颞下颌关节盘变性移位5例;MRI检查检出颞下颌关节盘变性移位61例,非颞下颌关节盘变性移位6例.MRI诊断颞下颌关节盘变性移位的准确度为95.52%(64/67)、灵敏度为 96.77%(60/62)、特异度为 80.00%(4/5)、阳性预测值为 98.36%(60/61)、阴性预测值为 66.67%(4/6),且MRI诊断结果与临床综合诊断结果一致性良好(Kappa值=0.703).MRI征象结果显示,颞下颌关节盘变性移位患者的可复性关节盘前移位、关节盘变性、关节腔积液检出率高于非颞下颌关节盘变性移位患者,且髁突前后径、左右径测量值小于非颞下颌关节盘变性移位患者(P<0.05).结论 MRI对颞下颌关节盘变性移位具有较高的诊断效能,可清晰显示颞下颌关节软组织及骨性结构的异常改变,能够为颞下颌关节盘变性移位的临床筛查与确诊提供可靠的影像学依据.
Objective To explore the diagnostic value of magnetic resonance imaging(MRI)for temporomandibular joint disc degeneration and displacement,and analyze its imaging features.Methods A total of 67 patients with suspected temporomandibular joint disc degeneration and displacement admitted to Lanzhou Second People's Hospital from January 2025 to June 2025 were selected as research subjects,and all patients underwent MRI examination.Taking the comprehensive clinical diagnosis as the gold standard,the efficacy of MRI in diagnosing temporomandibular joint disc degeneration and displacement was evaluated,and the differences in MRI signs between patients with and without temporomandibular joint disc degeneration and displacement were compared.Results Among the 67 patients,62 cases were confirmed as temporomandibular joint disc degeneration and displacement and 5 cases as non-temporomandibular joint disc degeneration and displacement by comprehensive clinical diagnosis;MRI examination detected 61 cases of temporomandibular joint disc degeneration and displacement and 6 cases of non-temporomandibular joint disc degeneration and displacement.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of MRI in diagnosing temporomandibular joint disc degeneration and displacement were 95.52%(64/67),96.77%(60/62),80.00%(4/5),98.36%(60/61)and 66.67%(4/6),respectively.The MRI results showed good consistency with comprehensive clinical diagnosis(Kappa value=0.703).MRI signs indicated that the detection rates of reducible anterior joint disc displacement,joint disc degeneration and joint cavity effusion in patients with temporomandibular joint disc degeneration and displacement were higher than those in patients without temporomandibular joint disc degeneration and displacement,and the measured values of anteroposterior and left-right diameters of the condyle were smaller than those in patients without temporomandibular joint disc degeneration and displacement(P<0.05).Conclusions MRI has high diagnostic efficacy for temporomandibular joint disc degeneration and displacement,and can clearly visualize the abnormal changes of soft tissue and bony structures of the temporomandibular joint,providing a reliable imaging basis for clinical screening and definitive diagnosis of temporomandibular joint disc degeneration and displacement.
