Comparative Study on Static Intensity Modulated Radiotherapy and Volumet-ric Modulated Arc Therapy in the Treatment of Nasopharyngeal Carcinoma

目的 对比分析静态调强放疗(s-IMRT)与容积旋转调强放疗(VMAT)治疗鼻咽癌的临床效果.方法 回顾性分析2024年11月至2025年12月我院收治的78例鼻咽癌患者的病例资料,根据放疗方式不同分为s-IMRT组(35例,采用s-IMRT治疗)和VMAT组(43例,采用VMAT治疗),两组均连续放疗6周.比较两组的临床缓解率、靶区剂量学参数、肿瘤标志物水平及不良反应发生率.结果 两组的临床总缓解率比较,差异无统计学意义(P>0.05).VMAT组的原发肿瘤靶区(PGTV)适形度指数(CI)明显高于s-IMRT组(P<0.05);两组的PGTV平均剂量、最大剂量、最小剂量、不均度指数(HI)比较,差异无统计学意义(P>0.05).VMAT组治疗后的血清糖类抗原125(CA125)、细胞角蛋白19片段(Cyfra21-1)、巨噬细胞炎性蛋白-3α(MIP-3α)水平均明显低于s-IMRT组(P<0.05).VMAT组的口腔黏膜炎、放射性皮炎、白细胞下降、放射性中耳炎发生率均明显低于s-IMRT组(P<0.05).结论 s-IMRT与VMAT两种方法均能有效治疗鼻咽癌,但VMAT在提高靶区适形度、下调相关肿瘤标志物水平及减少放疗急性不良反应方面更具优势,临床应用更为安全可靠.

Objective To compare and analyze the clinical efficacy of static intensity modulated radiotherapy(s-IMRT)and volumetric modulated arc therapy(VMAT)in the treatment of nasopharyngeal carcinoma.Methods A retrospective analysis was performed on the clinical data of 78 patients with nasopharyngeal carcinoma admitted to our hospital from November 2024 to December 2025.Patients were divided into the s-IMRT group(35 cases,treated with s-IMRT)and the VMAT group(43 cases,treated with VMAT)according to different radiotherapy modalities,with consecutive radiotherapy for 6 weeks in both groups.The clinical remission rate,dosimetric parameters of target volume,levels of tumor markers and incidence of adverse reactions were compared between the two groups.Results No statistically significant difference was found in the overall clinical remission rate between the two groups(P>0.05).The conformity index(CI)of the primary gross tumor volume(PGTV)in the VMAT group was significantly higher than that in the s-IMRT group(P<0.05).No significant differences were found in the mean dose,maximum dose,minimum dose,and heterogeneity index(HI)of the PGTV between the two groups(P>0.05).After treatment,the serum levels of carbohydrate antigen 125(CA125),cytokeratin 19 fragment(Cyfra21-1),and macrophage inflammatory protein-3α(MIP-3α)in the VMAT group were significantly lower than those in the s-IMRT group(P<0.05).The incidences of oral mucositis,radiation dermatitis,leukopenia,and radiation-induced otitis media in the VMAT group were significantly lower than those in the s-IMRT group(P<0.05).Conclusions Both s-IMRT and VMAT are effective therapeutic modalities for nasopharyngeal carcinoma.However,VMAT offers advantages in improving target conformity,reducing relevant tumor marker levels,and decreasing acute radiation-induced adverse reactions,making it a safer and more reliable option for clinical application.
