Legitimate Outsiders:An Anthropological Study of the"Doctor"Title Struggle in Traditional Chinese Medicine in Ontario,Canada
加拿大安大略省中医组织和中医师为争取"医生(Doctor或Dr.)"职业头衔而展开长期的抗争,这一争议涉及象征资本、专业合法性与医疗话语权之间的复杂关系.研究采用人类学方法,结合田野调查与政策文本的话语分析,聚焦多方相关利益者,包括中医从业者及患者.研究发现,该争议体现出一种"合法接纳-象征排斥"的治理逻辑:国家一方面通过立法将中医纳入正规医疗体系,实现其制度层面的接纳;另一方面却通过对象征资本的严格管控,维系以西医为中心的知识等级秩序与话语霸权.由此,中医虽名义上获得合法地位,实则被整合进一个受限制的制度体系中,成为"合法的局外人".
Traditional Chinese Medicine(TCM)practitioners and organizations in Ontario,Canada have waged a longstanding struggle to obtain the professional title of"Doctor"(or"Dr.").This controversy involves complex interactions among symbolic capital,professional legitimacy,and discursive power in medicine.Drawing on anthropological methods,including ethnographic fieldwork and discourse analysis of policy texts,this study focuses on multiple stakeholders including TCM practitioners and patients.The findings reveal a governance logic of"legitimate inclusion yet symbolic exclusion".On the one hand,the state incorporates TCM into the formal healthcare system through legislation,thereby granting institutional recognition.On the other hand,it strictly regulates access to symbolic capital in order to sustain a biomedical-centered hierarchy of knowledge and authority.As a result,TCM has obtained nominal legitimacy but has been integrated into a restricted institutional framework,becoming a"legitimate outsider".
