成纤维细胞生长因子21与2型糖尿病患者肾脏微血管病变机制的相关性研究OA
Association of fibroblast growth factor 21 with renal microvascular disease in patients with type 2 diabetes mellitus
目的 探讨成纤维细胞生长因子21(FGF21)水平与2型糖尿病(T2DM)患者肾脏微血管病变发生风险的关系,以及探索胰高血糖素样肽-1受体激动剂(GLP-1 RA)对这些参数的影响.方法 随机选取2021年11月至2022年12月在该科门诊就诊或住院的T2DM 患者64例为试验组,另选取同期在该院进行健康体检的人群32例为对照组,采用尿微量白蛋白与尿肌酐比值(UACR)评估肾脏微血管病变和肾功能.收集2组参与者的一般资料、生物标志物、实验室指标等检查结果,并应用描述性统计、t检验、χ2 检验,单因素和多因素线性回归及logistic回归模型等方法进行统计学分析,同时根据试验组患者是否使用 GLP-1 RA 进行亚组分析.结果 试验组FGF21和 UACR 水平[(203.20±44.89)pg/mL、2.4(1.3,6.5)mg/mmol]均显著高于对照组[(112.40±19.23)pg/mL、0.6(0.5,1.2)mg/mmol],差异均有统计学意义(P<0.001);试验组身体质量指数(BMI)及谷丙转氨酶、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)水平均显著高于对照组,差异均有统计学意义(P<0.05).多因素线性回归分析结果显示,FGF21水平与BMI(B=3.91,P<0.001)及log10转换后的 UACR(B=13.46,P=0.004)呈正相关,与谷草转氨酶呈负相关(B=-1.78,P<0.001).单因素线性回归分析显示,FGF21水平与TG、LDL-C及log10转换后的 UACR均呈正相关(P=0.005、0.010、<0.001).多因素logistic回归分析结果显示,T2DM 患者肾脏微血管病变与 FGF21显著相关且为风险因素(比值比=1.03,95%可信区间1.01~1.05,P=0.004).线性回归结果和logistic回归分析结果显示,使用GLP-1 RA与肾脏微血管病变发生风险及FGF21均呈负相关但不显著(P>0.05).结论 FGF21水平是 T2DM 患者发生肾脏微血管病变的风险因素,GLP-1 RA可能通过调节FGF21水平发挥对血管的保护作用.
Objective To investigate the association between fibroblast growth factor 21(FGF21)levels and the risk of renal microvascular disease in patients with type 2 diabetes mellitus(T2DM),and to explore the effects of glucagon-like peptide-1 receptor agonist(GLP-1RA)on these parameters.Methods A total of 64 T2DM patients who visited the outpatient clinic or were hospitalized in this department from November 2021 to December 2022 were randomly selected as the experimental group,and 32 healthy individuals who un-derwent physical examination at this hospital during the same period were selected as the control group.The urinary albumin-to-creatinine ratio(UACR)was used to assess renal microvascular disease and renal function.General data,biomarkers,and laboratory test results were collected from both groups.Statistical analyses were performed using descriptive statistics,t-test,chi-square test,univariate and multivariable linear regression,and logistic regression models.Subgroup analysis was also conducted based on whether patients in the experimen-tal group received GLP-1RA.Results The FGF21 and UACR levels in the experimental group[(203.20±44.89)pg/mL,2.4(1.3,6.5)mg/mmol]were significantly higher than those in the control group[(112.40±19.23)pg/mL,0.6(0.5,1.2)mg/mmol],with statistically significant differences(P<0.05).The body mass index(BMI),alanine aminotransferase,triglyceride(TG),and low-density lipoprotein cholesterol(LDL-C)lev-els in the experimental group were significantly higher than those in the control group,with statistically sig-nificant differences(P<0.05).Multivariate regression showed FGF21 positively correlated with BMI(B=3.91,P<0.001)and log10-UACR(B=13.46,P=0.004),and negatively with aspartate aminotransferase(B=1.78,P<0.001).Univariate regression showed positive correlations with TG,LDL-C,and log10-UACR(P=0.005,0.010,and<0.001,respectively).Multivariable logistic regression analysis showed that renal mi-crovascular disease in T2DM patients was significantly associated with FGF21 and was also a risk factor(odds ratio=1.03,95%confidence interval 1.01-1.05,P=0.004).Linear regression and logistic regression analy-ses revealed that GLP-1RA use was negatively but non-significantly associated with the risk of renal microvas-cular disease and FGF21 levels(P>0.05).Conclusion FGF21 level is a risk factor for renal microvascular disease in T2DM patients.GLP-1RA may exert vascular protective effects by modulating FGF21 levels.
