基于NHANES的美国人群NPAR与脑卒中风险关联研究OA
Association between neutrophil percentage-to-albumin ratio and stroke risk in the US Population based on NHANES
目的 探究美国人群中性粒细胞百分比与白蛋白比值(NPAR)与脑卒中风险之间的潜在关联,为脑卒中的预防和控制提供依据.方法 选择2009-2020年美国健康和营养检查调查数据库中20岁以上美国成年人作为研究对象,测量 NPAR.采用加权多因素logistic回归研究 NPAR和脑卒中风险的相关性,使用多变量线性回归模型和限制性三次样条(RCS)曲线模型评估NPAR与脑卒中风险之间的关系.结果 共纳入28 815名研究对象,平均年龄为(49.67±17.61)岁,其中脑卒中患者共1 157例,脑卒中检出率为4.02%(1 157/28 815).多因素logistic回归分析显示,连续性 NPAR与脑卒中呈正相关[比值比(OR)=1.05,95%可信区间(95%CI)1.01~1.09,P<0.01].与 Q1比 较,Q2、Q3、Q4个体发生脑卒中的可能性分别增加54%(OR=1.54,95%CI 1.13~2.10,P<0.05)、41%(OR=1.41,95%CI 1.02~1.93,P<0.05)、75%(OR=1.75,95%CI 1.23~2.48,P<0.05).RCS曲线显示,美国成年人脑卒中风险和 NPAR二者之间呈非线性关系(P非线性<0.05).阈值效应分析显示,NPAR与脑卒中风险的关系在12.8处存在显著拐点(P<0.05).亚组分析显示,种族不同分层之间存在交互作用.结论 NPAR值与美国成年人脑卒中风险之间呈非线性正相关关系,高NPAR值是美国成年人发生脑卒中的危险因素.
Objective To investigate the potential association between the neutrophil percentage-to-al-bumin ratio(NPAR)and stroke risk in the US population,thereby providing evidence for stroke prevention and control.Methods US adults aged 20 years and older from the National Health and Nutrition Examination Survey(NHANES)2009-2020 were selected as study subjects,and NPAR was measured.Weighted multiva-riable logistic regression was used to examine the correlation between NPAR and stroke risk,and multivari-able linear regression models and restricted cubic spline(RCS)curves were employed to evaluate the associa-tion.Results A total of 28 815 participants were included,with a mean age of(49.67±17.61)years.Among them,1 157 had a stroke,yielding a stroke prevalence of 4.02%(1 157/28 815).Multivariate logistic regres-sion analysis showed that continuous NPAR was positively associated with stroke[odds ratio(OR)=1.05,95%confidence interval(CI)1.01-1.09,P<0.01].Compared with Q1,the risk of stroke in individuals in the Q2,Q3,and Q4 increased by 54%(OR=1.54,95%CI 1.13-2.10,P<0.05),41%(OR=1.41,95%CI 1.02-1.93,P<0.05),and 75%(OR=1.75,95%CI 1.23-2.48,P<0.05),respectively.The RCS curve re-vealed a nonlinear relationship between NPAR and stroke risk in US adults(P for nonlinearity<0.05).Threshold effect analysis showed a significant inflection point at an NPAR of 12.8(P<0.05).Subgroup anal-ysis showed that there was an interaction among different racial strata.Conclusion There is a nonlinear posi-tive association between NPAR and stroke risk in US adults,and a high NPAR is a risk factor for stroke in this population.
