呼吸训练器联合综合康复训练在呼吸重症患者中的应用效果分析OA
Application Effect of Respiratory Trainer Combined with Comprehensive Rehabilitation Training in Patients with Severe Respiratory Disease
目的 探讨呼吸训练器联合综合康复训练在呼吸重症患者中的应用效果.方法 选取2023-2024年新疆医科大学第一附属医院呼吸重症监护病房收治的呼吸重症患者88例为研究对象,采用随机数字表法将患者分为对照组与观察组,各44例.对照组进行综合康复训练,观察组在对照组的基础上联合呼吸训练器进行训练,两组均训练1个月.比较两组训练前、训练1个月后呼吸功能指标(平均气道压、气道峰压、动态顺应性)、氧合功能指标(动脉血氧分压、动脉血二氧化碳分压、血氧饱和度)、功能独立性评定量表(FIM)各维度得分及训练期间并发症发生率.结果 训练1个月后,观察组平均气道压、气道峰压低于对照组,动态顺应性高于对照组(P<0.05);对照组、观察组训练1个月后平均气道压、气道峰压分别低于本组训练前,动态顺应性分别高于本组训练前(P<0.05).训练1个月后,观察组动脉血氧分压、血氧饱和度高于对照组,动脉血二氧化碳分压低于对照组(P<0.05);对照组、观察组训练1个月后动脉血氧分压、血氧饱和度分别高于本组训练前,动脉血二氧化碳分压分别低于本组训练前(P<0.05).训练1个月后,观察组社会认知、转移、自理能力、行走、交流、控制括约肌维度得分高于对照组(P<0.05);对照组、观察组训练1个月后社会认知、转移、自理能力、行走、交流、控制括约肌维度得分分别高于本组训练前(P<0.05).两组训练期间并发症总发生率比较,差异无统计学意义(P>0.05).结论 呼吸训练器联合综合康复训练在呼吸重症患者中有较好的应用效果,其可有效改善患者呼吸功能和氧合功能,提高神经肌肉功能,且安全性较好.
Objective To investigate the application effect of respiratory trainer combined with comprehensive rehabilitation training in patients with severe respiratory disease.Methods A total of 88 patients with severe respiratory disease admitted to the Respiratory Intensive Care Unit of the First Affiliated Hospital of Xinjiang Medical University from 2023 to 2024 were selected as the study subjects.Using the random number table method,the patients were divided into the control group and the observation group,with 44 cases in each group.The control group received comprehensive rehabilitation training,while the observation group performed training using respiratory trainer in addition to the regimen of the control group.Both groups were trained for 1 month.The respiratory function indicators(mean airway pressure,peak inspiratory pressure,dynamic compliance),oxygenation function indicators(arterial partial pressure of oxygen,arterial carbon dioxide partial pressure,oxygen saturation),and the scores of each dimension of Functional Independence Measure(FIM)before training and after 1 month of training,and the incidence of complications during training were compared between the two groups.Results After 1 month of training,the mean airway pressure and peak inspiratory pressure in the observation group were significantly lower than those in the control group,while the dynamic compliance was significantly higher than that in the control group(P<0.05).Compared with before training,both groups showed significantly decreased mean airway pressure and peak inspiratory pressure,and significantly increased dynamic compliance after 1 month of training(P<0.05).After 1 month of training,arterial partial pressure of oxygen and oxygen saturation in the observation group were significantly higher than those in the control group,whereas the arterial carbon dioxide partial pressure was significantly lower than that in the control group(P<0.05).Compared with before training,both groups had significantly increased arterial partial pressure of oxygen and oxygen saturation,and significantly decreased arterial carbon dioxide partial pressure after 1 month of training(P<0.05).After 1 month of training,the scores of social cognition,transfers,self-care,locomotion,communication,and sphincter control dimensions in the observation group were significantly higher than those in the control group(P<0.05).Compared with before training,both groups demonstrated significantly increased scores of these dimensions after 1 month of training(P<0.05).There was no statistically significant difference in the total incidence of complications during training between the two groups(P>0.05).Conclusion Respiratory trainer combined with comprehensive rehabilitation training have high application effect in patients with severe respiratory disease.It can effectively improve patients'respiratory function and oxygenation function,enhance neuromuscular function,and demonstrate a high safety profile.
