首发脑卒中恢复期患者症状群及核心症状的同期网络分析OA
Synchronization Network Analysis of Symptom Clusters and Core Symptoms in Patients with First Stroke during Recovery Period
目的 调查首发脑卒中恢复期患者症状发生率及严重程度,分析其症状群及核心症状.方法 2025年4-9月,采用便利抽样法在陕西中医药大学附属医院选取脑卒中恢复期患者作为调查对象.采用脑卒中症状体验量表调查患者症状体验情况.通过探索性因子分析提取症状群,采用R 4.5.1和R-Studio构建同期网络并通过中心性指标(包括强度中心性、紧密中心性和中介中心性)确定核心症状.结果 本研究共发放问卷298份,回收有效问卷298份,回收有效率为100.0%.首发脑卒中恢复期患者症状发生率为23.8%~99.3%,其中发生率位居前5的症状分别是因不能做想做的事情烦恼、疲乏、闷闷不乐、容易着急、肢体无力,其中因不能做想做的事情烦恼是频率、严重度、困扰度得分相对较高的症状.提取的4个症状群分别是躯体活动症状群、认知障碍症状群、下肢运动症状群、疼痛症状群,累积方差贡献率为74.974%.同期网络分析结果显示,足下垂和自理能力下降为症状群的核心症状.结论 躯体活动症状群、认知障碍症状群、下肢运动症状群及疼痛症状群是首发脑卒中恢复期患者的4大症状群,其中足下垂和自理能力下降是其症状群的核心症状,建议医护人员基于症状群和核心症状制订干预措施,从而提高症状管理效率.
Objective To investigate the incidence and severity of symptoms in patients with first stroke during recovery period,and to construct their symptom clusters and core symptoms.Methods From April to September in 2025,the stroke patients during recovery period were selected as the investigation subjects by convenient sampling method in the Affiliated Hospital of Shaanxi University of Chinese Medicine.The Symptom Experience Scale of Stroke was used to investigate the symptom experience of patients.The symptom clusters were extracted by exploratory factor analysis.R 4.5.1 and R-Studio were used to construct the synchronization network,and the core symptoms were determined by centrality indicators(including strength,closeness and betweenness).Results A total of 298 questionnaires were distributed in this study,and 298 valid questionnaires were recovered,with an effective recovery rate of 100.0%.The incidence of symptoms in patients with first stroke during recovery period was 23.8%-99.3%.The top 5 symptoms were annoyed by not being able to do what you want to do,fatigue,depressed,easy to worry,and limb weakness.Among them,annoyed by not being able to do what you want to do was the symptom with relatively high score of frequency,severity,and distress.The 4 symptom clusters extracted were physical activity symptom cluster,cognitive impairment symptom cluster,lower limb motor symptom cluster and pain symptom cluster,with a cumulative variance contribution rate of 74.974%.The results of synchronization network analysis showed that foot drop and decreased self-care ability were the core symptoms of symptom clusters.Conclusion Physical activity symptom cluster,cognitive impairment symptom cluster,lower limb motor symptom cluster and pain symptom cluster are the 4 major symptom clusters in patients with first stroke during recovery period.Among them,foot drop and decreased self-care ability are the core symptoms of the symptom clusters.It is suggested that medical staff should formulate intervention measures based on symptom clusters and core symptoms,so as to improve the efficiency of symptom management.
