卵巢型子宫内膜异位症患者血清及腹腔冲洗液中Tim-3和Gal-9的表达及意义OA
Expression and Significance of Tim-3 and Gal-9 in Serum and Peritoneal Lav-age Fluid of Patients with Ovarian Endometriosis
目的:检测卵巢型子宫内膜异位症(EMT)患者血清及腹腔冲洗液中 T 细胞免疫球蛋白黏蛋白-3(Tim-3)和半乳糖凝集素-9(Gal-9)的表达,探讨二者在评估卵巢型 EMT 疾病进展以及非侵入性诊断中的潜在价值.方法:选取2024 年3 月至12 月在昆明医科大学第二附属医院进行手术治疗的卵巢型 EMT 患者37 例为病例组,选择同期因输卵管良性疾病因素(输卵管堵塞、输卵管积水)而致不孕及卵巢良性肿瘤需行手术治疗的患者37 例为对照组.收集两组患者术前空腹静脉血、术中腹腔冲洗液,通过酶联免疫吸附试验(ELISA)检测 Tim-3、Gal-9 的表达.分析 Tim-3、Gal-9 在病例组和对照组之间的表达差异,探讨卵巢型 EMT 患者中二者的表达与疾病进展的关系,并绘制受试者工作特征(ROC)曲线分析血清中Tim-3、Gal-9 单独检测与联合检测对卵巢型EMT 的诊断效能.结果:卵巢型 EMT 患者血清及腹腔冲洗液中 Tim-3、Gal-9 的表达均显著高于对照组,差异有统计学意义(P<0.05).卵巢型 EMT 患者美国生殖医学学会(ASRM)分期Ⅲ~Ⅳ期(重型)血清及腹腔冲洗液中 Tim-3、Gal-9 的表达水平高于 I~Ⅱ期(轻型),差异有统计学意义(P<0.05),而二者的表达与患者有无痛经无明显关系(P>0.05).卵巢型 EMT 患者腹腔冲洗液中 Tim-3、Gal-9 的表达均明显高于血清,差异有统计学意义(P<0.001).ROC 分析结果显示血清 Tim-3 及 Gal-9 诊断卵巢型 EMT的截断值分别为64.62 pg/ml、3.74 pg/ml,曲线下面积(AUC)分别为 0.698、0.672,敏感度分别为64.9%、67.6%,特异度分别为75.7%、70.3%;二者联合诊断的 AUC 为0.726,敏感度为75.7%、特异度为62.2%,其诊断效能高于 Tim-3 或 Gal-9 单独检测(P<0.05).结论:Tim-3 和 Gal-9 在卵巢型 EMT 的免疫调控机制中有重要作用,其表达与 EMT 的解剖学严重程度相关,且对卵巢型 EMT 的诊断具有一定的价值,有望成为非侵入性诊断该疾病以及评估疾病进展的潜在生物标志物.
Objective:To detect the expressions of T-cell immunoglobulin mucin-3(Tim-3)and galectin-9(Gal-9)in the serum and peritoneal lavage fluid of patients with ovarian endometriosis(EMT),and to explore their potential value for evaluating the disease progression and non-invasive diagnosis of ovarian EMT.Methods:37 patients with ovarian EMT who underwent surgical treatment at The Second Affiliated Hospital of Kunming Medical University from March to December 2024 were selected as the case group,and 37 patients with infertility caused by benign fallopian tube disease(fallopian tube obstruction,hydrosalpinx)and benign ovarian tumors re-quiring surgical treatment during the same period were selected as the control group.The preoperative fasting ve-nous blood and intraoperative peritoneal lavage fluid of the two groups of patients were collected,and the expres-sions of Tim-3 and Gal-9 were detected by enzyme-linked immunosorbent assay(ELISA).We analyzed the ex-pression differences of Tim-3 and Gal-9 between the case group and the control group,explore the relationship between the expression of the two and disease progression in patients with ovarian EMT,and draw the receiver operating characteristic(ROC)curve to analyze the diagnostic efficacy of single and combined detection of Tim-3 and Gal-9 in serum for ovarian EMT.Results:The expressions of Tim-3 and Gal-9 in serum and peritoneal lavage fluid of ovarian EMT patients were significantly higher than those of the control group,and the differences were statistically significant(P<0.05).The expression levels of Tim-3 and Gal-9 in serum and peritoneal lavage fluid of patients with ovarian EMT in ASRM stage Ⅲ-Ⅳ were higher than those in ASRM stage Ⅰ-Ⅱ,and the difference was statistically significant(P<0.05).There was no significant relationship between the expression of TIM-3 and Gal-9 and dysmenorrhea(P>0.05).The expressions of Tim-3 and Gal-9 in peritoneal lavage fluid of ovarian EMT patients were significantly higher than those in serum,and the differences were statistically significant(P<0.001).ROC analysis showed that the cut-off value of serum Tim-3 and Gal-9 in the diagnosis of ovarian EMT was 64.62 pg/ml and 3.74 pg/ml,respectively,the area under the curve(AUC)was 0.698 and 0.672,respectively,and the sensitivity was 64.9%and 67.6%,respectively.The specificities of the two indexes were 75.7%and 70.3%;The AUC of the combination of Tim-3 and Gal-9 was 0.726,the sensitivity was 75.7%,and the specificity was 62.2%,which was inferior to the diagnostic efficiency of Tim-3 or Gal-9 alone(P<0.05).Conclusions:Tim-3 and Gal-9 play important roles in the immune regulatory mechanism of ovarian EMT.Their expressions are related to the an-atomical severity of EMT and have certain value for the diagnosis of ovarian EMT.They are expected to become potential biomarkers for non-invasive diagnosis of this disease and assessment of disease progression.
