目的:探讨血清可溶性 E-钙黏蛋白(sE-cadherin)、前梯度蛋白2(AGR2)与ⅠA1~ⅡA2 期子宫颈癌患者根治术后复发的相关性.方法:选取2021 年1 月至2023 年6 月于新疆医科大学第一附属医院接受腹腔镜根治术治疗的133 例ⅠA1~ⅡA2 期子宫颈癌患者临床资料进行回顾性分析,根据其术后随访2 年期间肿瘤复发情况分为复发组(n=28)与未复发组(n=105).比较两组患者一般资料和术前实验室指标差异情况;通过 Cox 回归分析ⅠA1~ⅡA2 期子宫颈癌患者根治术后复发的影响因素;通过限制性立方样条法(RCS)及受试者工作特征(ROC)曲线分析血清 sE-cadherin、AGR2 水平与ⅠA1~ⅡA2 期子宫颈癌患者根治术后复发的剂量-反应关系及其预测效能.结果:复发组淋巴结转移、脉管瘤栓占比及血清细胞角蛋白19 片段(CYFRA21-1)、sE-cadherin、AGR2 水平均高于未复发组,差异有统计学意义(P<0.05).Cox 回归分析显示,淋巴结转移、脉管瘤栓、血清sE-cadherin 及 AGR2 均为ⅠA1~ⅡA2 期子宫颈癌患者根治术后复发的影响因素(P<0.05).RCS分析发现,血清 sE-cadherin、AGR2 水平与ⅠA1~ⅡA2 期子宫颈癌患者根治术后复发呈非线性剂量-反应关系(P<0.05).ROC 曲线分析发现,血清sE-cadherin、AGR2 单独及联合检测对ⅠA1~ⅡA2 期子宫颈癌患者根治术后复发均有一定预测效能,且联合预测效能更高.结论:血清 sE-cadherin、AGR2水平与ⅠA1~ⅡA2 期子宫颈癌患者根治术后复发有关,可辅助临床早期预测患者术后复发风险.

Objective:To investigate the correlation between serum soluble E-cadherin(sE-cadherin),anterior gradient protein 2(AGR2),and postoperative recurrence in patients with stage ⅠA1-ⅡA2 cervical cancer who received radical surgery.Method:The clinical data of 133 patients with stage ⅠA1-ⅡA2 cervical cancer who un-derwent laparoscopic radical hysterectomy at the First Affiliated Hospital of Xinjiang Medical University from Janu-ary 2021 to June 2023 were retrospective analyzed.According to tumor recurrence during the 2-year postoperative follow-up,the patients were divided into the recurrence group(n=28)and the non-recurrence group(n=105).General clinical data and preoperative laboratory indicators were compared between the two groups.The influen-cing factors of postoperative recurrence in patients with stage ⅠA1 to ⅡA2 cervical cancer were analyzed by Cox regression analysis.The dose-response relationship and predictive efficacy of serum sE-cadherin and AGR2 lev-els and recurrence after radical resection in patients with stage ⅠA1 to ⅡA2 cervical cancer after radical surgery were analyzed by restricted cubic spline method(RCS)and receiver operating characteristic(ROC)curve.Results:The proportion of lymph node metastasis,vascular tumor thrombus,and serum levels of CYFRA21-1,sE-cadherin,and AGR2 in the recurrent group were higher than those in the non recurrent group,with statistically significant differences(P<0.05).Cox regression analysis showed that lymph node metastasis,vascular tumor thrombus,serum sE-cadherin,and AGR2 were influencing factors for postoperative recurrence in patients with stage ⅠA1-ⅡA2 cervical cancer after radical surgery(P<0.05).RCS analysis revealed a non-linear dose-re-sponse relationship between serum levels of sE-cadherin and AGR2 and postoperative recurrence in patients with stage ⅠA1-ⅡA2 cervical cancer after radical surgery(P<0.05).ROC curve analysis showed that serum sE-cadherin and AGR2,both individually and in combination,had a certain predictive power for postoperative re-currence in patients with stage ⅠA1-ⅡA2 cervical cancer after radical surgery,and the combined predictive power was higher.Conclusions:Serum levels of sE-cadherin and AGR2 are associated with postoperative recur-rence in patients with stage ⅠA1-ⅡA2 cervical cancer after radical surgery.The two indicators can assist in ear-ly clinical prediction of postoperative recurrence risk.
