老年细菌性肺炎病人28 d病死率与老年营养风险指数的相关性OA
Correlation between 28-day mortality and the geriatric nutritional risk index in elderly patients with bacterial pneumonia
目的:研究老年细菌性肺炎病人不良结局与老年营养风险指数(GNRI)的相关性.方法:基于 MIMIC-Ⅳ 2.2数据库选取2 903例老年细菌性肺炎病人作为研究对象.基于GNRI将营养风险分为4个等级.采用限制性立方样条法(RCS)、Kaplan-Meier回归分析、Cox比例风险回归模型、亚组分析阐明老年细菌性肺炎病人28 d病死率与GNRI的关系,采用Boruta算法对28 d死亡率预测变量进行重要性排序.结果:老年细菌性肺炎病人28 d死亡率与GNRI呈非线性负相关关系(P<0.001),GNRI=90.281为临床干预临界点.GNRI升高与死亡率降低相关.Cox比例风险回归模型结果显示,GNRI为病人28 d死亡率的重要预测因子(P<0.001).亚组分析显示,人口统计学资料、合并症等因素对28 d死亡率与GNRI间关系影响不明显(P>0.05).GNRI为28 d死亡率的关键影响因素之一.结论:GNRI对老年细菌性肺炎病人短期死亡率具有一定预测价值,治疗期间补充营养可能降低死亡率.
Objective:To investigate the correlation between adverse outcomes and the Geriatric Nutritional Risk Index(GNRI)in elderly patients with bacterial pneumonia.Methods:Based on the MIMIC-Ⅳ 2.2 database,a total of 2 903 elderly patients with bacterial pneumonia were selected as the study subjects.Nutritional risk was classified into four grades according to GNRI.The relationship between 28-day mortality and GNRI in elderly patients with bacterial pneumonia was elucidated using restricted cubic splines(RCS),Kaplan-Meier regression analysis,Cox proportional hazards regression models,and subgroup analysis.The Boruta algorithm was employed to rank the importance of predictors for 28-day mortality.Results:A non-linear negative correlation was observed between 28-day mortality and GNRI in elderly patients with bacterial pneumonia(P<0.001).GNRI value of 90.281 was identified as the critical point for clinical intervention.An elevated GNRI was associated with reduced mortality.The results of the Cox proportional hazards regression model indicated that GNRI was a significant predictor of 28-day mortality(P<0.001).Subgroup analysis revealed that factors such as demographic characteristics and comorbidities did not have statistically significant impact on the relationship between 28-day mortality and GNRI(P>0.05).GNRI was identified as one of the key influencing factors for 28-day mortality.Conclusions:GNRI has demonstrated predictive value for short-term mortality in elderly patients with bacterial pneumonia.Nutritional support for elderly patients with bacterial pneumonia during treatment may reduce their mortality.
