心电图碎裂QRS波对急性心肌梗死患者发生室性心律失常的预测价值OA

Objective:To analyze the predictive value of QRS wave of ECG fragmentation on ventricular arrhythmia in acute myocardial infarction.Methods:Sixty patients with acute myocardial infarction admitted to Mingzhou Hospital in Ningbo from January 2024 to June 2025 were examined by routine 12-lead electrocardiogram,and were divided into fragmented QRS wave group(n=30)and non-fragmented QRS wave group(n=30)according to the QRS wave shape of patients.24-hour dynamic electrocardiogram was performed in both groups to analyze the occurrence of ventricular arrhythmia.By drawing ROC curve,the predictive value of QRS wave of ECG fragmentation on ventricular arrhythmia in patients with a-cute myocardial infarction was analyzed.Results:The incidence of ventricular arrhythmia in the fragmented QRS group was 90.00%(27/30),which was significantly higher than that in the non-fragmented QRS group(53.33%,16/30)(P<0.05).Compared with non-fragmented QRS complex,the levels of PNN50,rMSSD and SDNN in fragmented QRS complex were lower(P<0.05).The ROC curve analysis results showed that the fractured QRS complex in electrocardio-gram had a certain predictive value for ventricular arrhythmias in patients with acute myocardial infarction,with an AUC value of 0.726(P<0.05),a Youden's index of 0.452,a sensitivity of 0.628,and a specificity of 0.824.Conclusion:The incidence of ventricular arrhythmia in patients with acute myocardial infarction with fragmented QRS wave is higher than that in patients with non-fragmented QRS wave,and the indexes of heart rate variability are obviously decreased.The QRS wave of ECG fragmentation has a good prediction effect on the occurrence of ventricular arrhythmia in patients,which can be popularized and applied.
