超声造影联合血管内皮生长因子、γ-谷氨酰转肽酶对原发性肝癌患者肝动脉化疗栓塞术后疗效评价的价值研究OA
Value of Contrast-enhanced Ultrasound Combined with Vascular Endothelial Growth Factor And γ-glutamyl Transpeptidase in Evaluating The Curative Effect of Patients with Primary Liver Cancer after Transcatheter Arterial Chemoembolization
目的 分析超声造影(CEUS)联合血管内皮生长因子(VEGF)、γ-谷氨酰转肽酶(GGT)应用于原发性肝癌(HCC)患者肝动脉化疗栓塞术(TACE)后疗效评价中的价值.方法 选取2024年7月至2025年6月在湛江中心人民医院诊治的 HCC患者100例,所有患者均在术前1周及术后1个月、3个月开展 CEUS检查,同时检测血清VEGF、GGT水平,并在术后3个月,根据实体瘤疗效评价标准评估其临床疗效,分为有效组(完全缓解+部分缓解,n=64)和无效组(疾病稳定+疾病进展,n=36).对比两组术前及术后不同时间点 CEUS相关参数、血清VEGF及GGT水平,分析各指标与临床疗效的相关性,以及联合检测对 TACE的诊断效能.结果 术后1个月、3个月,有效组血流灌注强度评分低于无效组,病灶缩小率高于无效组(P<0.05).术后1个月、3个月,有效组血清VEGF、GGT水平均低于无效组(P<0.05).相关性分析发现,术后3个 月,CEUS血流灌注强度、血清 VEGF及GGT水平与病灶缩小率表现为负相关(r=-0.682、-0.715、-0.653,P<0.05).CEUS联合 VEGF、GGT检测评价TACE疗效的灵敏度、特异度、准确率均高于单一指标检测(P<0.05).结论 CEUS联合血清VEGF、GGT检测能够全面反映 HCC患者在TACE后的肿瘤血液供应变化、血管生成及肝功能状态,有助于及时调整患者的治疗方案.
Objective:To analyze the value of contrast-enhanced ultrasound(CEUS)combined with vascular endo-thelial growth factor(VEGF)and γ-glutamyltranspeptidase(GGT)in evaluating the curative effect after tran-scatheter arterial chemoembolization(TACE)in patients with primary liver cancer(HCC).Methods:100 patients with HCC who were diagnosed and treated in Zhanjiang Central People's Hospital from July 2024 to June 2025 were selected.All patients underwent CEUS examination one week before operation and one month and three months af-ter operation,and their serum VEGF and GGT levels were detected at the same time.Three months after opera-tion,their clinical efficacy was evaluated according to the evaluation criteria of solid tumor efficacy,and they were divided into effective group(complete remission+partial remission,n=64)and ineffective group(stable disease)CEUS-related parameters,serum VEGF and GGT levels were compared between the two groups at different time points before and after operation,and the correlation between each index and clinical efficacy was analyzed,as well as the diagnostic efficiency of joint detection for TACE.Results:One month and three months after operation,the score of blood perfusion intensity in the effective group was lower than that in the ineffective group,and the lesion reduction rate was higher than that in the ineffective group(P<0.05).One month and three months after opera-tion,the levels of serum VEGF and GGT in the effective group were lower than those in the ineffective group(P<0.05).Correlation analysis showed that 3 months after operation,CEUS blood perfusion intensity,serum VEGF and GGT levels were negatively correlated with the lesion reduction rate(r=-0.682,-0.715,-0.653,P<0.05).The sensitivity、specificity and accuracy of CEUS combined with VEGF and GGT in evaluating the efficacy of TACE were higher than those of single index(P<0.05).Conclusion:CEUS combined with serum VEGF and GGT can comprehensively reflect the changes of tumor blood supply,angiogenesis and liver function in HCC pa-tients after TACE,which is helpful to adjust the treatment plan of patients in time.
