目的:观察炙甘草汤加味联合西药对症治疗对心肌梗死后心律失常患者氧化应激及炎症水平的影响.方法:选取2024年1月—2025年3月河南中医药大学第一附属医院心内科收治的心肌梗死后心律失常患者106例,分为对症组53例(对症治疗)和汤剂组53例(炙甘草汤加味+对症治疗),两组连续治疗4周.比较两组患者心电图指标、CXC趋化因子配体8(CXCL8)、肿瘤坏死因子-α(TNF-α)、半乳糖凝集素3(Gal-3)、白细胞介素-6(IL-6)、核因子E2 相关因子2(NRF2)、B型脑钠肽(BNP)、丙二醛(MDA)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)以及不良反应的发生率.结果:两组患者治疗前心电图指标比较,差异无统计学意义(P>0.05);治疗后两组校正QT间期(QTc)、QRS时限、QT离散度(QTd)均较治疗前降低,且汤剂组治疗后心电图指标低于对症组(P<0.05).治疗前两组各项观察指标比较,差异无统计学意义(P>0.05);治疗后两组CXCL8、Gal-3、MDA、TNF-α、IL-6、BNP水平均较治疗前降低,SOD、NRF2、GSH-Px水平均较治疗前升高,且治疗后汤剂组上述指标均优于对症组,差异均具有统计学意义(P<0.05,P<0.01).两组不良反应发生率(汤剂组7.55%,对症组11.32%)比较,差异无统计学意义(P>0.05).结论:炙甘草汤加味联合西药对症治疗可减轻心肌梗死后心律失常患者氧化应激反应,抑制炎症,改善心电图.

Objective:To observe the effect of modified Zhigancao Decoction combined with symptomatic western medicine on oxidative stress and inflammatory levels in patients with post-myocardial infarction arrhythmias.Methods:A to-tal of 106 patients with post-myocardial infarction arrhythmias admitted to the Department of Cardiac Function Examination of the First Affiliated Hospital of Henan University of Chinese Medicine from January 2024 to March 2025 were selected and divided into a symptomatic group(53 cases,receiving symptomatic treatment alone)and a decoction group(53 cases,receiv-ing modified Zhigancao Decoction plus symptomatic treatment).Both groups were treated continuously for 4 weeks.The two groups were compared in terms of electrocardiographic parameters,levels of CXC chemokine ligand 8(CXCL8),tumor ne-crosis factor-alpha(TNF-α),galectin-3(Gal-3),interleukin-6(IL-6),nuclear factor erythroid 2-related factor 2(NRF2),B-type brain natriuretic peptide(BNP),malondialdehyde(MDA),superoxide dismutase(SOD),glutathione peroxidase(GSH-Px),as well as the incidence of adverse reactions.Results:There was no statistically significant difference in electro-cardiographic parameters between the two groups before treatment(P>0.05).After treatment,corrected QT interval(QTc),QRS duration,and QT dispersion(QTd)decreased in both groups compared with before treatment,and the electrocardio-graphic parameters in the decoction group were lower than those in the symptomatic group after treatment(P<0.05).Before treatment,there were no statistically significant differences in any of the observed indicators between the two groups(P>0.05).After treatment,the levels of CXCL8,Gal-3,MDA,TNF-α,IL-6,and BNP decreased in both groups compared with before treatment,while the levels of SOD,NRF2,and GSH-Px increased compared with before treatment.Moreover,after treatment,the above indicators in the decoction group were superior to those in the symptomatic group,with statistically sig-nificant differences(P<0.05,P<0.01).There was no statistically significant difference in the incidence of adverse reactions between the two groups(7.55%in the decoction group vs.11.32%in the symptomatic group)(P>0.05).Conclusion:Modi-fied Zhigancao Decoction combined with symptomatic western medicine can reduce oxidative stress,inhibit inflammation,and improve electrocardiographic parameters in patients with post-myocardial infarction arrhythmias.
