不同分子分型乳腺癌的钼靶与动态对比增强磁共振成像影像特征分析及检出率比较OA
Mammography and Dynamic Contrast-Enhanced MRI:Imaging Features and Detection Rate Comparison across Breast Cancer Molecular Subtypes
目的:比较钼靶与动态对比增强磁共振成像(dynamic contrast-enhanced MRI,DCE-MRI)在不同分子分型乳腺癌中的影像学特征差异,并评价两者单独及并行联合对已确诊乳腺癌的检出率.方法:回顾性纳入 2019 年 10 月至2024 年 9 月在我院就诊且完成钼靶、DCE-MRI 检查与病理分析证实的女性乳腺肿瘤患者 120 例.按免疫组化分型标准分为 Luminal A、Luminal B(非 HER2 阳性)、HER2 阳性(非 Luminal 型)、三阴性乳腺癌,比较钼靶与 DCE-MRI特征分布;统计各方法在各分型中的检出例数与检出率.结果:不同分子分型乳腺癌的钼靶及 DCE-MRI 特征分布存在差异.Luminal A、Luminal B 及 HER2 阳性亚型中,3 种检测方法检出率总体比较差异无统计学意义(P>0.05).三阴性亚型中 3 种方法检出率总体比较差异有统计学意义(P<0.05);进一步两两比较采用 McNemar 精确检验并行Bonferroni 校正(α=0.017)显示,仅钼靶与联合应用差异有统计学意义(P<0.017).结论:钼靶与 DCE-MRI 在不同分子分型乳腺癌中呈现差异化影像特征,并行联合可提高乳腺癌的总体检出率,三阴性分型获益更为明显.
Objective:To compare the differences in imaging features between mammography and dynamic contrast-en-hanced MRI(DCE-MRI)among different molecular subtypes of breast cancer,and to evaluate the detection rates of the two modalities when used alone or in combination for pathologically confirmed breast cancer.Methods:We retrospectively en-rolled 120 female patients with breast neoplasms confirmed by pathology who received mammography and DCE-MRI examina-tions from October 2019 to September 2024.Tumors were categorized according to immunohistochemical criteria into Luminal A,Luminal B(non-HER2-positive),HER2-positive(non-Luminal),and triple-negative breast cancer subtypes.The ima-ging features of mammography and DCE-MRI were compared across subtypes,and the detected case numbers and detection rates of the two modalities in each subtype were calculated.Results:Imaging features of mammography and DCE-MRI dif-fered across molecular subtypes.For Luminal A,Luminal B,and HER2-positive tumors,overall detection rates did not dif-fer among the three strategies(mammography,DCE-MRI,and the combination of the two)(P>0.05).For triple-negative tumors,overall detection rates differed significantly(P<0.05);pairwise comparisons using exact McNemar test with Bonferroni correction(α=0.017)showed that a statisti-cally significant difference existed only between mammography and the combined strategy(P<0.017).Conclusion:Mam-mography and DCE-MRI exhibit distinct imaging features across different molecular subtypes of breast cancer;combined ap-plication improves overall detection rates for pathologically confirmed breast cancer,with the greatest benefit observed in tri-ple-negative subtype.
