目的:目前缺乏适用于我国多发性骨髓瘤(multiple myeloma,MM)患者的生命质量测评工具.本研究旨在基于癌症患者生命质量测定量表共性模块(Quality of Life Instruments for Cancer Patients-General Module Version 2.0,QLICP-GM V2.0),构建MM 患者生命质量量表(Quality of Life Instruments for Patients with MM,QLICP-MM)的特异模块.方法:以QLICP-GM V2.0 为基础,采用共性模块与特异性模块相结合的方式,通过议题小组讨论提出条目池,结合多次核心小组讨论对条目进行筛选形成初步的量表,抽取出 30 名 MM 患者及 20 名医生进行问卷调查及访谈,采用变异度法、相关系数法、克朗巴赫系数(Cronbach's α)法和医患重要性评分法进行条目筛选,筛选阈值包括:相关系数>0.50、变异度>0.90、删除条目后 Cronbach's α 不升高、患者重要性评分≥70 分、医生重要性评分≥80 分.结果:共 16 个候选条目进入分析.条目变异度范围为 0.85~1.60;条目-维度相关系数为 0.24~0.78;大多数条目Cronbach's α 删除后均未升高(稳定在 0.84~0.87);患者重要性评分:65.33~90.33 分;医生重要性评分:77.50~92.50 分.根据统计学结果保留 10 个条目,经专家讨论后最终确定 11 个条目作为 QLICP-MM 特异模块测试版.结论:本研究基于严格的定性与小样本定量方法构建了 QLICP-MM 特异模块.最终保留的 11 个条目具有良好的内容效度和内部一致性,为进一步形成正式版量表及其临床验证奠定了基础.

Objective:Currently,there is a lack of appropriate quality-of-life assessment tools for patients with multiple myeloma(MM)in China.This study aimed to devel-op the disease-specific module of the Quality of Life Instru-ments for Patients with MM(QLICP-MM),based on the Quality of Life Instruments for Cancer Patients-General Module(Version 2.0)(QLICP-GM V2.0).Methods:Using QLICP-GM V2.0 as the basic framework,this study adopted a com-bined approach of a general module and a disease-specific module.An initial item pool was generated through topic-group dis-cussions,followed by several rounds of core-group reviews to develop the preliminary scale.A total of 30 patients with MM and 20 medical professionals participated in questionnaires and interviews.Item screening was performed using the variation meth-od,correlation coefficient method,Cronbach's α method,and clinician-patient importance rating method.Screening thresh-olds were set as follows:correlation coefficient>0.50,coefficient of variation>0.90,no increase in Cronbach's α after item deletion,patient importance score≥70 points,and medical staff importance score≥80 points.Results:A total of 16 candi-date items were included in the analysis.The coefficient of variation for each item ranged from 0.85 to 1.60;item-dimension correlation coefficients ranged from 0.24 to 0.78.Cronbach's α remained stable(0.84~0.87)after item deletion.Patient importance scores ranged from 65.33 to 90.33,and medical professional importance scores ranged from 77.50 to 92.50.Based on the statistical results,10 items were retained.After expert consultation,11 items were finalized for the test version of the QLICP-MM disease-specific module.Conclusion:Using a rigorous process combining qualitative and small-sample quantitative methods,this study developed the disease-specific module of QLICP-MM.The final 11 items show good content validity and in-ternal consistency,laying a foundation for the future development of the formal scale and its clinical validation.
