目的:基于连续病理切片,建立三阴性乳腺癌(triple-negative breast cancer,TNBC)癌巢的三维重建与量化体系,解析其空间聚集规律与形态谱系.方法:纳入 5 例 TNBC 手术病理标本,连续切片、HE 染色并数字化全景扫描.采用面向细胞的数据分析进行多尺度配准、DeepLabv3+完成语义分割;按像素间距构建体素栈,并以 26 邻域界定"独立癌巢",获得尺度可比的三维连通体.于该统计单元上量化体积(V)、球形度(ψ)、表面积/体积比(SA/V);以三维 Ripley 的 L(r)-r 评估质心在不同尺度 r 的空间偏离,并以稳健 z 分数热图概括样本级"形态-尺度"型谱.结果:二维癌巢个数均显著高于三维"独立计数",二维癌巢与三维癌巢总比为197 113∶27 427(约为7.2∶1),该差异表明癌巢在三维空间中具有独立性.L(r)-r 显示:癌巢质心约在10 μm 由离散转为聚集,至25~30 μm 仍为正,提示短—中程成簇为稳健特征且强度存在个体差异.合并层面,ψ 主要分布于 0.60~0.80(中位≈0.65),表面积/体积比(SA/V)集中在 0.60~1.00 μm-1(中位≈0.70 μm-1),在体积可比时 ψ 与 SA/V 负相关,形成由"更圆整、表面起伏较少"到"不规则、起伏较多"的连续谱系(包括如板层化、环形伴中央腔隙、桥连与孔洞化以及网状多孔等);样本热图呈数量驱动与体积驱动两端型谱,并见高 SA/V 大巢尾部亚群.结论:基于三维连通体定义的独立癌巢,二维与三维癌巢之比证实三维癌巢具有独立性,并揭示TNBC 在约10 μm起始的短程聚集及几何形态谱系(如板层化、环形伴中央腔隙、桥连与孔洞化以及网状多孔等).使用三维独立计数、聚集的起点与强度,叠加球形度与表面积/体积比,样本分类为体积驱动型、数量驱动型以及混合型,共同构成一套可解释且可复用的三维量化框架.

Objective:To build a three-dimensional(3D)reconstruction and quantification pipeline for cancer nests in triple-negative breast cancer(TNBC)from serial histological sections and characterize their spatial clustering patterns and morphological spectrum.Methods:Five surgical pathology specimens of TNBC were included.These specimens underwent serial sectioning,hematoxylin and eosin staining,and whole-slide imaging digitization.Cellular oriented data analysis was employed for multi-scale registration,and DeepLabv3+was uti-lized for semantic segmentation.A voxel stack was constructed based on pixel spacing,and"discrete cancer nests"were de-fined using 26-neighborhood connectivity to obtain scale-comparable 3D connected volumes.Within these statistical units,morphological parameters including volume,sphericity(ψ),and surface area-to-volume ratio(SA/V)were quantified.Spa-tial deviation of centroids across different scales was assessed using the 3D Ripley's L(r)-r function,and sample-level"morphology-scale"profiles were summarized using a robust z-score heatmap.Results:The two-dimensional(2D)cancer nest count was significantly higher than the 3D"independent count"(197,113 vs 27,427,ratio ≈ 7.2∶1),which indi-cates that cancer nests are spatially independent in 3D space.L(r)-r analysis showed that the centroids of cancer nests shif-ted from discrete distribution to clustered aggregation at approximately 10 μm;the values remained positive within 25~30 μm,indicating that short-and medium-range clustering constitutes a robust feature with interindividual variation in cluste-ring intensity.Analysis of merged data showed ψ values primarily distributed between 0.60~0.80(median ≈ 0.65),while SA/V were concentrated in the 0.60~1.00 μm-1 range(median ≈ 0.70 μm-1).At comparable volumes,ψ and SA/V exhibited a negative correlation,forming a continuous morphological spectrum from"more spherical with minimal sur-face irregularity"to"highly irregular with complex surface contours"(including laminar organization,annular structures with central lumina,bridging with fenestrations,and reticulated porous patterns).Sample-level heat maps revealed distinct quantity-driven and volume-driven profiles,along with a tail subpopulation characterized by large nests with high SA/V rati-os.Conclusion:The 3D-connected volume-defined independent cancer nests,verified by the 2D-to-3D cancer nest ratio,confirm the independence of 3D cancer nests,and reveal short-range clustering starting at approximately 10 μm in TNBC as well as a geometric morphological spectrum(including laminar organization,annular structures with central lumina,bridging with fenestrations,and reticulated porous patterns).By incorporating 3D independent counting,clustering initiation and in-tensity,ψ,and surface area-to-volume ratios,we classified samples into volume-driven,quantity-driven,and mixed sub-types,collectively constructing an interpretable and reusable 3D quantitative framework.
