基于症状管理理论脑卒中老年患者口腔衰弱管理方案的构建OA
Development of an oral frailty management program for elderly stroke patients based on Symptom Management Theory
目的 以症状管理理论为框架,构建科学、可行的老年脑卒中患者口腔衰弱管理方案.方法 通过文献研究和半结构式访谈,建构老年脑卒中患者口腔衰弱管理方案初稿,并邀请18名专家进行2轮德尔菲专家函询,形成方案终稿.结果 2轮专家函询有效回收率100％,专家权威系数分别为0.878、0.932.最终方案包括:3项一级条目、14项二级条目和40项三级条目,各条目重要性赋值为4.72～5.00分,变异系数为0～0.104,专家肯德尔和谐系数(W)为0.142(P<0.001).结论 本研究结合症状管理理论,提出了符合老年脑卒中患者口腔衰弱特点的重要口腔症状,如口角歪斜、流涎、口齿不清、咀嚼困难、吞咽困难等.基于症状管理理论构建的老年脑卒中患者口腔衰弱管理方案内容科学、结构清晰,具有较好的科学性和可行性,能为神经科开展老年脑卒中患者口腔衰弱管理提供参考.
Objective To develop a scientific and feasible oral frailty management program for elderly stroke patients based on Symp-tom Management Theory(SMT).Methods Through literature review and semi-structured interviews,a preliminary draft of the man-agement program for oral frailty in elderly stroke patients was developed.Two rounds of Delphi expert consultation were conducted with 18 experts to finalize the program.Results The response rate for both rounds of expert consultation was 100％,with expert authority coefficients of 0.878 and 0.932,respectively.The final program comprised 3 first-level,14 second-level,and 40 third-level items.The importance scores for each item ranged from 4.72 to 5.00,with coefficients of variation ranging from 0 to 0.104,and the Kendall's coeffi-cient of concordance(W)was 0.142(P<0.001).Conclusion This study identifies key oral symptoms of oral frailty in elderly stroke patients,including facial asymmetry,drooling,slurred speech,difficulty chewing,and difficulty swallowing.The oral frailty manage-ment program for elderly stroke patients,developed based on SMT,is clearly structured,and scientifically valid and feasible.It can serve as a reference for neurology departments in implementing oral frailty management for elderly stroke patients.
