Effects of productive dust and noise exposure on hyperglycemia in automobile manufacturing workers

[背景]生产性粉尘与噪声均是职业有害因素,二者均能引起劳动者血糖变化. [目的]探究生产性粉尘与噪声暴露的交互作用对汽车制造工人高血糖患病风险的影响. [方法]采用整群抽样的方法,选取 2022年武汉市 3家汽车制造厂职业健康体检工人作为研究对象,并采用中国疾病预防控制中心设计的调查表收集工人基本信息,接触职业病危害因素情况来自职业健康检查中职业史问诊资料,血压、血脂、血糖、脂肪肝等指标来自职业健康检查.采用空腹血糖高于6.1 mmol·L-1 为高血糖判定标准.使用 logistic回归模型分析职业性噪声与粉尘暴露对高血糖患病风险的影响.采用相加、相乘交互作用模型分析生产性粉尘与噪声暴露对高血糖患病的交互作用. [结果]1 223名研究对象高血糖的检出率为 7.7%.logistic回归分析结果显示,相较于非粉尘非噪声暴露的工人,仅粉尘暴露、仅噪声暴露和二者同时暴露的工人患高血糖的比值比(OR)及其 95%置信区间(CI)分别为 2.15(1.90~4.23)、2.23(1.40~5.08)、3.75(1.95~7.24).噪声接害工龄与血糖浓度存在线性关联(P<0.05).交互作用分析结果显示,粉尘和噪声暴露对高血糖无相乘交互作用,也不存在相加交互作用. [结论]生产性粉尘和噪声暴露是影响高血糖的危险因素,二者存在联合作用,但无交互作用.

[Background]Both productive dust and noise are occupational hazards,and each can induce changes in blood glucose levels among workers. [Objective]To explore the potential interactive effects of exposure to productive dust and noise on the risk of hyperglycemia among automobile manufacturing workers. [Methods]Using a cluster sampling method,workers undergoing occupational health examination from three automobile manufacturing plants in Wuhan in 2022 were selected.Basic information was collected using a questionnaire designed by the Chinese Center for Disease Control and Pre-vention.Exposure history was obtained through occupational history interviews.Clinic indicators such as blood pressure,blood lipids,fasting blood glucose(FBG),and fatty liver status,were re-trieved from examination records.Hyperglycemia was defined as FBG>6.1 mmol·L-1.Logistic re-gression models analyzed the independent effects of occupational noise and dust exposure,while additive and multiplicative interaction models assessed their interactive effects on hyper-glycemia. [Results]The prevalence of hyperglycemia among the 1 223 participants was 7.7%.Logistic re-gression showed that using workers without exposure as reference,the odds ratios(OR)and 95%confidence intervals(CI)for hyperglycemia among workers exposed only to dust,only to noise,and to both were 2.15(1.90,4.23),2.23(1.40,5.08),and 3.75(1.95,7.24),respectively.A linear association was observed between the duration of noise exposure and blood glucose levels(P<0.05).Interaction analysis showed no multiplicative interaction or additive interaction. [Conclusion]Exposure to productive dust and noise are independent risk factors for hyperglycemia among workers.While both contribute to increased risk,no significant synergistic interaction is observed.
