目的 探究根盾技术即刻种植在上颌前牙区患牙修复中的应用效果.方法 选取2020年4月—2023年1月在张家口市第一医院口腔科行上颌前牙区即刻种植的患者22例,根据随机数字表法分为不翻瓣组(常规不翻瓣即刻种植修复)及根盾组(根盾技术即刻种植修复),每组11例.比较两组术后36个月种植体留存率、种植体不同位点硬组织吸收量[术后即刻与术后36个月唇侧硬组织厚度(FBW)、骨总厚度(BW)及唇侧骨高度(FBH)的差值(ΔFBW、ΔBW、ΔFBH)]、种植体软组织变化[粉红色美学评分(PES)、唇侧轮廓突度变化、唇侧龈缘变化]、满意度及并发症发生率.结果 截至末次随访,22颗种植体稳定,无显著骨吸收,均能正常行使功能,两组种植体存留率均为100%(11/11),差异无统计学意义(P>0.05).根盾组在肩台0 mm水平及种植体根中位点的ΔFBW及ΔBW均低于不翻瓣组,且ΔFBH低于不翻瓣组(P<0.05).两组术后口内彩照及扫描PES比较差异无统计学意义(P>0.05),但根盾组唇侧轮廓突度变化和唇侧龈缘变化均小于不翻瓣组(P<0.05).两组满意度及并发症发生率比较差异无统计学意义(P>0.05).结论 根盾技术即刻种植用于上颌前牙区患牙修复具有一定的临床效果,能有效减轻唇侧骨板的吸收和软组织退缩,在改善美学效果及患者满意度方面也存在一定的积极作用,且未提高并发症发生率.

Objective To investigate the application effect of immediate implantation using socket-shield technique(SST)in the restoration of diseased teeth in the anterior maxilla.Methods Twenty-two patients who underwent immediate implantation in the anterior maxilla at Department of Stomatology,the First Hospital of Zhangjiakou City from April 2020 to January 2023 were selected.They were divided into the flapless group(conventional flapless immediate implant restoration)and the socket-shield group(immediate implant restoration using SST)according to the random number table method,with 11 patients in each group.The implant survival rate at 36 months postoperatively,hard tissue resorption at different sites of the implants[the difference in the facial bone width(FBW),total bone width(BW),and facial bone height(FBH)immediately after surgery and at 36 months postoperatively(ΔFBW,ΔBW,ΔFBH)],the changes in the soft tissue of the implants[pink esthetic score(PES),changes in the labial contour prominence,changes in the labial gingival margin],patient satisfaction,and the incidence of complications were compared between the two groups.Results As of the last follow-up,all 22 implants were stable with no significant bone resorption,and all were able to function normally.The implant survival rate in both groups was 100%(11/11),and the difference was not statistically significant(P>0.05).The ΔFBW and ΔBW at the 0-mm horizontal level of the abutment shoulder and the mid-root level of the implant in the socket-shield group were lower than those in the flapless group,and the ΔFBH was also lower than that in the flapless group(P<0.05).There was no significant difference in the intraoral photographs and PES scans between the two groups(P>0.05),but the changes in the labial contour prominence and the labial gingival margin position in the root-membrane group were smaller than those in the flapless group(P<0.05).There was no significant difference in patient satisfaction and complication rates between the two groups(P>0.05).Conclusion Immediate implantation using SST for immediate implantation in the restoration of diseased teeth in the anterior maxilla has promising clinical effectiveness.It can effectively reduce labial bone plate resorption and soft tissue recession,while also contributing to improved aesthetic outcomes and patient satisfaction,without increasing the incidence of complications.
