生肌玉红膏联合解毒洗剂治疗糖尿病足气阴两虚证临床效果观察OA
Clinical effect of Shengji Yuhong ointment combined with Jiedu lotion on diabetic foot with Qi and Yin deficiency syndrome
目的 探讨生肌玉红膏联合解毒洗剂治疗糖尿病足气阴两虚证的效果.方法 回顾性分析2022年12月—2025年2月平顶山市中医医院收治的60例糖尿病足患者临床资料,根据治疗方法分为对照组、观察组各30例.对照组采取常规治疗,观察组在对照组基础上联合生肌玉红膏及解毒洗剂治疗.比较两组患者临床疗效、不良反应、创面愈合情况(首次出现肉芽组织时间与创面愈合时间)、治疗前后创面面积与中医证候总积分变化、治疗前后C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)等炎症反应指标变化及血管内皮生长因子(VEGF)、碱性成纤维生长因子(bFGF)及内皮抑素(ES)水平.结果 观察组总有效率[100.00％(30/30)]较对照组[80.00％(24/30)]高(P＜0.05).两组不良反应发生率比较差异无统计学意义(P＞0.05).观察组创面首次出现肉芽组织与创面愈合时间均较对照组短(P＜0.01).治疗后,两组患者创面面积、中医证候总积分及CRP、IL-6、TNF-α、ES水平均较治疗前下降,且观察组低于对照组(P＜0.05,P＜0.01).两组治疗后VEGF及bFGF水平较治疗前升高,且观察组较对照组高(P＜0.05).结论 生肌玉红膏联合解毒洗剂可较好促进糖尿病足气阴两虚证患者创面愈合,疗效显著,不良反应少.
Objective To investigate the therapeutic effect of Shengji Yuhong ointment combined with Jiedu lotion in the treatment of diabetic foot with Qi and Yin deficiency syndrome.Methods A retrospective analysis was conducted on the data of 60 diabetic foot patients admitted to Pingdingshan Hospital of Traditional Chinese Medicine from December 2022 to February 2025.According to different treatment methods,they were divided into the control group and the observation group.The control group received conventional treatment,while the observation group was supplemented with Shengji Yuhong ointment and Jiedu lotion on the basis of the control group.The clinical effect,adverse reactions,wound healing conditions(the time to the first appearance of granulation tissue and the time to wound healing),changes in wound area and total score of TCM syndromes before and after treatment,changes in inflammatory response indicators such as C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)before and after treatment,and levels of vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF),and endostatin(ES)was compared between the two groups.Results The total effective rate of the observation group[100.00％(30/30)]was higher than that of the control group[80.00％(24/30)](P＜0.05).There was no significant difference in adverse reactions between the two groups(P＞0.05).The time to the first appearance of granulation tissue on the wound surface and the time to wound healing in the observation group were shorter than those in the control group(P＜0.01).After treatment,the wound area,total score of TCM syndrome scores,CRP,IL-6,TNF-α,and ES levels of both groups decreased compared with those before treatment,which were smaller or lower in the observation group than in the control group(P＜0.05,P＜0.01).The levels of VEGF and bFGF in the two groups after treatment increased compared with those before treatment,which were higher in the observation group than in the control group(P＜0.05).Conclusion Shengji Yuhong ointment combined with Jiedu lotion can better promote the wound healing of diabetic foot patients with Qi and Yin deficiency syndrome,with significant curative effect and fewer adverse reactions.
