目的 探讨骨髓间充质干细胞来源外泌体(BMSCs-EXO)对慢性阻塞性肺疾病(COPD)大鼠肺功能及沉默信息调节因子1(SIRT1)/核因子-κB(NF-κB)信号通路的影响.方法 采用脂多糖联合熏烟方式构建COPD大鼠模型.将大鼠分为对照组、COPD组、COPD+EXO组、COPD+EXO+EX527组(EX527为SIRT1抑制剂),每组8只.BMSCs-EXO经雾化吸入给药.使用小动物肺功能仪测试各组肺功能指标,HE染色评估大鼠肺组织病理损伤,Masson染色评估大鼠肺组织纤维化情况,酶联免疫吸附试验检测大鼠血清白细胞介素(IL)-6、IL-8、肿瘤坏死因 子(TNF)-α水平,蛋白质印迹法检测大鼠肺组织中SIRT1、核因子-κB抑制蛋白α(IκBα)蛋白的表达及磷酸化(p)-NF-κB p65)/NF-κB p65.结果 相较于对照组,COPD组大鼠的肺功能指标显著降低(P<0.05),肺组织呈现明显的病理学损伤,肺组织的胶原纤维面积百分比显著增加(P<0.05),血清IL-6、IL-8、TNF-α水平显著升高(P<0.05),肺组织SIRT1、IκBα蛋白表达水平显著降低(P<0.05),p-NF-κB p65/NF-κB p65显著升高(P<0.05).相较于 COPD组,COPD+EXO 组大鼠的肺功能指标显著升高(P<0.05),肺组织的病理损伤减轻,肺组织的胶原纤维面积百分比显著降低(P<0.05),血清IL-6、IL-8、TNF-α水平显著降低(P<0.05),肺组织SIRT1、IκBα蛋白表达水平显著升高(P<0.05),p-NF-κB p65/NF-κB p65显著降低(P<0.05).相较于COPD+EXO组,COPD+EXO+EX527组大鼠的肺功能指标显著降低(P<0.05),肺组织的病理损伤加剧,肺组织的胶原纤维面积百分比显著升高(P<0.05),血清IL-6、IL-8、TNF-α的水平显著升高(P<0.05),肺组织SIRT1、IκBα蛋白表达水平显著降低(P<0.05),p-NF-κB p65/NF-κB p65升高(P<0.05).结论 BMSCs-EXO可能通过调控SIRT1/NF-κB信号通路来抑制炎症,进而改善COPD大鼠的肺功能.

Objective To investigate the effects of bone marrow mesenchymal stem cell-derived exosomes(BMSCs-EXO)on pulmonary function and the silent information regulator 1(SIRT1)/nuclear factor-κB(NF-κB)signaling pathway in rats with chronic obstructive pulmonary disease(COPD).Methods A rat model of COPD was established by combined lipopolysaccharide and cigarette smoke exposure.Rats were divided into a control group,a COPD group,a COPD+EXO group and a COPD+EXO+EX527 group(EX527 is a SIRT1 inhibitor),with 8 rats in each group.BMSCs-EXO were administered via aerosol inhalation.Pulmonary func-tion parameters of rats in each group were measured using a small animal pulmonary function testing system.Histopathological injury of lung tissue was evaluated by HE staining,and pulmonary fibrosis was assessed by Masson staining.Serum levels of interleukin(IL)-6,IL-8 and tumor necrosis factor(TNF)-α were detected by enzyme-linked immunosorbent assay.The protein expressions of SIRT1 and nuclear factor-κB inhibitor alpha(IκBα),as well as the ratio of phosphorylated(p)-NF-κB p65 to NF-κB p65 in rat lung tissue,were determined by Western blotting.Results Compared with the control group,pulmonary function parameters in the COPD group were significantly decreased(P<0.05),obvious pathological injury of lung tissue was observed,the percentage of collagen fiber area in lung tissue was significantly increased(P<0.05),serum levels of IL-6,IL-8 and TNF-α were significantly elevated(P<0.05),the protein expression levels of SIRT1 and IκBα in lung tissue were significantly decreased(P<0.05),and the ratio of p-NF-κB p65 to NF-κB p65 was significantly increased(P<0.05).Compared with the COPD group,pulmonary function parameters in the COPD+EXO group were significantly increased(P<0.05),pathological injury of lung tissue was alleviated,the percentage of collagen fiber area in lung tissue was significantly decreased(P<0.05),serum levels of IL-6,IL-8 and TNF-α were significantly decreased(P<0.05),the protein expression levels of SIRT1 and IκBα in lung tissue were significantly increased(P<0.05),and the ratio of p-NF-κB p65 to NF-κB p65 was significantly de-creased(P<0.05).Compared with the COPD+EXO group,pulmonary function parameters in the COPD+EXO+EX527 group were significantly decreased(P<0.05),pathological injury of lung tissue was exacerba-ted,the percentage of collagen fiber area in lung tissue was significantly increased(P<0.05),serum levels of IL-6,IL-8 and TNF-α were significantly elevated(P<0.05),the protein expression levels of SIRT1 and IκBα in lung tissue were significantly decreased(P<0.05),and the ratio of p-NF-κB p65 to NF-κB p65 was signifi-cantly increased(P<0.05).Conclusion BMSCs-EXO may improve pulmonary function in rats with COPD by inhibiting inflammation through the regulation of the SIRT1/NF-κB signaling pathway.
