首发缺血性脑卒中病人急性应激障碍现状及影响因素研究OA
Status quo and influencing factors of acute stress disorder in patients with first-stage ischemic stroke
目的:探究首发缺血性脑卒中(ischemic stroke,IS)病人发生急性应激障碍的现状及影响因素.方法:选取天津市某三级甲等医院388例首发IS病人为研究对象,根据其是否发生急性应激障碍(acute stress disorder,ASD)分为ASD组和非ASD组,采用一般资料调查表、斯坦福急性应激反应问卷、心理韧性量表、领悟社会支持量表、简易应对方式量表进行调查,并通过单因素及多因素分析病人发生ASD的影响因素.结果:388例病人中有179例病人发生ASD,发生率为46.13%;Logistic回归分析显示,性格、吞咽功能障碍、偏瘫、睡眠障碍、心理韧性、社会支持是首发IS病人发生ASD的独立影响因素(P<0.05).结论:首发IS病人ASD的发生率较高,其发生与病人的性格、吞咽功能障碍、偏瘫、睡眠障碍、心理韧性及社会支持水平有关,提示医护人员应尽早识别高危人群,并采取有效的措施进行预防及干预.
Objective:To investigate the status quo and influencing factors of acute stress disorder(ASD)in patients with first-stage ischemic stroke(IS).Methods:A total of 388 patients with first-stage IS from a tertiary hospital in Tianjin were selected as the study subjects.They were divided into ASD group and non-ASD group based on whether they had ASD.A general information questionnaire,the Stanford Acute Stress Response Questionnaire(SASRQ),Connor-Davidson Resilience Scale(CD-RISC),Perceived Social Support Scale(PSSS),and Simplified Coping Style Questionnaire(SCSQ)were used for the survey.Univariate and multivariate analyses were conducted to identify the influencing factors of ASD in the patients.Results:Among 388 patients,179 developed ASD,with an incidence rate of 46.13%.Logistic regression analysis showed that personality,dysphagia,hemiplegia,sleep disturbances,psychological resilience,and social support were independent influencing factors for ASD in first-episode ischemic stroke(IS)patients(P<0.05).Conclusion:The incidence of ASD is high in first-episode IS patients,and its occurrence is related to the patient's personality,dysphagia,hemiplegia,sleep disturbances,psychological resilience,and level of social support.This suggests that medical staff should identify high-risk groups as early as possible and take effective measures for prevention and intervention.
