湖州市中小学生脊柱弯曲异常的现状及影响因素分析OA
Status and influencingfactors of abnormal spinal curvature among primary and middle school students in Huzhou City
目的 掌握浙江省湖州市中小学生脊柱弯曲异常的现况及影响因素,为制定中小学生脊柱弯曲异常的防控措施提供依据.方法 采用分层整群随机抽样方法,对湖州市 28 所学校 7 573 名中小学生进行脊柱弯曲异常的筛查和问卷调查.结果 湖州市中小学生脊柱弯曲异常检出率为 4.71%.其中,女生(5.42%)高于男生(4.01%);城区学生(6.43%)高于郊县学生(3.35%);高中学生(7.28%)高于小学(1.42%)和初中(6.09%),差异均有统计学意义(χ2=8.390、39.656、108.528,均 P<0.01).多因素 Logistic 回归分析结果显示,郊县(OR=0.588,95%CI:0.468~0.740)、周末或节假日每天≥60 min 中高强度运动(OR少数能做到=0.598,95%CI:0.368~0.973;OR一半日子能做到=0.693,95%CI:0.482~0.996;OR多数能做到=0.686,95%CI:0.476~0.991),以及班级座位左右轮转(OR=0.748,95%CI:0.562~0.995)是脊柱弯曲异常的保护因素(均 P<0.05);初中(OR=4.241,95%CI:2.959~6.079)和高中学生(OR=4.509,95%CI:3.063~6.638)更易发生脊柱弯曲异常(均 P<0.05).结论 湖州市中小学生脊柱弯曲异常检出率较高,应重视城区、初中和高中学生脊柱弯曲异常的防控.建议学生在周末或节假日进行中高强度运动、学校定期进行座位左右轮转,以降低中小学生脊柱弯曲异常的发生率.
Objective To investigate the current status of abnormal spinal curvature and its influencing factors among primary and middle school students in Huzhou City,Zhejiang Province,and to provide a basis for developing prevention and control measures.Methods A stratified cluster random sampling method was used to screen for abnormal spinal curvature and conduct a questionnaire survey among 7,573 primary and middle school students from 28 schools in Huzhou City.Results The pass rate of abnormal spinal curvature was 4.71%.The pass rate was higher in female students(5.42%)than in male students(4.01%),higher in urban students(6.43%)than in suburban students(3.35%),and higher in high school students(7.28%)than in primary school students(1.42%)and junior high school students(6.09%),with all differences being statistically significant(χ2=8.390,39.656,108.528,all P<0.01).Multivariate logistic regression analysis showed that suburban residence(OR=0.588,95%CI:0.468-0.740),moderate-to-vigorous physical activity for≥60 minutes per day on weekends or holidays(ORfor"a few days"=0.598,95%CI:0.368-0.973;ORfor"abouthalf of the days"=0.693,95%CI:0.482-0.996;ORfor"most days"=0.686,95%CI:0.476-0.991),and regular left-right rotation of classroom seats(OR=0.748,95%CI:0.562-0.995)were protective factors against abnormal spinal curvature(all P<0.05).Junior high school(OR=4.241,95%CI:2.959-6.079)and high school(OR=4.509,95%CI:3.063-6.638)were associated with a higher risk of abnormal spinal curvature(both P<0.05).Conclusions The pass rate of abnormal spinal curvature among primary and middle school students in Huzhou City is relatively high.Priority should be given to the prevention and control of abnormal spinal curvature among students in urban areas,as well as junior and senior high school students.It is recommended that students engage in moderate-to-vigorous physical activity on weekends or holidays,and that schools regularly rotate classroom seats from left to right,so as to reduce the incidence of abnormal spinal curvature in this population.
