中性粒细胞-淋巴细胞比值、降钙素原联合检测对儿童重症呼吸道合胞病毒肺炎的预测价值OA
Predictive value of combined detection of neutrophil-to-lymphocyte ratio and procalcitonin for severe respiratory syncytial virus pneumonia in children
目的 分析联合检测中性粒细胞-淋巴细胞比值(NLCR)、降钙素原(PCT)对儿童重症呼吸道合胞病毒(RSV)肺炎的预测价值.方法 选取 2023 年 3 月至 2025 年 1 月九江市妇幼保健院接受治疗的 100 例 RSV肺炎患儿作为研究对象,将其按照重症肺炎诊断标准分为重症 RSV 肺炎组(重症组,50 例)与非重症 RSV 肺炎组(非重症组,50 例).入院 24 h内,对两组患儿的淋巴细胞计数、中性粒细胞计数、白细胞计数及 PCT 水平进行测定,对NLCR水平进行计算.分析重症组与非重症组实验室指标、临床特征(临床表现、影像学表现),以及 NLCR、PCT联合检测预测效能指标.结果 重症组的淋巴细胞计数水平低于非重症组,中性粒细胞计数、白细胞计数、PCT、NL-CR水平高于非重症组,差异有统计学意义(P<0.05).重症组的次要临床表现、肺不张的占比高于非重症组,差异有统计学意义(P<0.05).PCT、NLCR联合检测诊断重症RSV肺炎的敏感度、特异度高于单独检测PCT或NLCR.结论 通过 PCT、NLCR水平联合检测能更准确地识别 RSV 感染患儿的重症化风险,对儿童重症 RSV 肺炎具有良好的预测价值.
Objective To analyze the predictive value of combined detection of neutrophil-to-lymphocyte ratio(NLCR)and procalcitonin(PCT)for severe respiratory syncytial virus(RSV)pneumonia in children.Methods A total of 100 children with RSV pneumonia who were treated at Jiujiang Maternal and Child Health Hospital from March 2023 to January 2025 were selected as the research subjects.They were divided into the severe RSV pneumonia group(severe group,50 cases)and the non-severe RSV pneumonia group(non-severe group,50 cases)according to the diagnostic criteria for severe pneumo-nia.Within 24 h of admission,the lymphocyte count,neutrophil count,white blood cell count,and PCT level of the two groups of children were measured,and the NLCR level were calculated.The laboratory indicators,clinical characteristics(clinical manifestations,imaging manifestations)of the severe group and the non-severe group,as well as the predictive efficacy indicators of the combined detection of NLCR and PCT were analyzed.Results The lymphocyte count level of the severe group was lower than that of the non-severe group,while the neutrophil count,white blood cell count,PCT,and NLCR levels were higher than those of the non-severe group,and the differences were statistically significant(P<0.05).The secondary clinical manifestations and the proportion of atelectasis in the severe group were higher than those in the non-severe group,and the differences were statistically significant(P<0.05).The sensitivity and specificity of combined detection of PCT and NLCR for diagnosing severe RSV pneumonia were higher than those of detecting PCT or NLCR alone.Conclusion The combined detection of PCT and NLCR levels can more accurately identify the risk of severe deterioration in children with RSV infection and has a good predictive value for severe RSV pneumonia in children.
