血清颗粒蛋白前体、正五聚蛋白3 水平对牙列缺失患者口腔种植修复术后种植体周围黏膜炎的预测价值OA
Serum progranulin,pentraxin 3 levels in patients with edentulism oral implant restoration of peri-implant mucositis after predictive value
目的 探讨血清颗粒蛋白前体(PGRN)、正五聚蛋白3(PTX3)水平对牙列缺失患者口腔种植修复术后种植体周围黏膜炎的预测价值.方法 选取2021年12月至2024年12月菏泽医学专科学校附属医院收治的153例进行口腔种植修复术患者作为研究对象,根据种植修复术后6个月牙列缺失患者种植体周围黏膜炎的发生情况,分为黏膜炎组(41例)和无黏膜炎组(112例).采用ELISA法检测血清PGRN、PTX3水平;采用多因素logistic分析牙列缺失患者术后发生种植体周围黏膜炎的影响因素;比较不同血清PGRN、PTX3水平对牙列缺失患者术后发生种植体周围黏膜炎的相对危险度;采用受试者工作特征(ROC)曲线评估血清PGRN、PTX3水平对牙列缺失患者术后发生种植体周围黏膜炎的预测价值.结果 黏膜炎组患者糖尿病、吸烟人数占比及血清PTX3水平高于无黏膜炎组,PGRN水平低于无黏膜炎组,差异有统计学意义(P<0.05).吸烟、糖尿病、PTX3为牙列缺失患者术后发生种植体周围黏膜炎的独立危险因素,PGRN为独立保护因素(P<0.05).PGRN≥95.58、PTX3≥12.64表达的牙列缺失患者术后发生种植体周围黏膜炎的相对危险度是PGRN<95.58、PTX3<12.64表达的患者的0.404倍和3.141倍(P<0.05).PGRN、PTX3联合预测牙列缺失患者术后发生种植体周围黏膜炎的曲线下面积(AUC)为0.945,优于PGRN、PTX3单一预测.结论 牙列缺失患者术后种植体周围黏膜炎患者血清PGRN水平降低,PTX3水平升高,二者联合预测牙列缺失患者术后发生种植体周围黏膜炎的价值较高.
Objective To explore the predictive value of serum progranulin(PGRN)and pentraxin 3(PTX3)levels for peri-implant mucositis in patients with edentulism after oral implant restoration.Methods A total of 153 patients who underwent oral implant restoration at Heze Medical College Affiliated Hospital from December 2021 to December 2024 were selected as the research subjects.According to the occurrence of peri-implant mucositis in patients with tooth loss 6 months after im-plant restoration,they were divided into the mucositis group(41 cases)and the non-mucositis group(112 cases).The levels of serum PGRN and PTX3 were detected by ELISA method.Multivariate logistic analysis was used to analyze the influencing factors of peri-implant mucositis in patients with tooth loss after surgery.The relative risk of peri-implant mucositis in patients with tooth loss after surgery for different serum PGRN and PTX3 levels was compared.The receiver operating characteristic(ROC)curve was used to evaluate the predictive value of serum PGRN and PTX3 levels for peri-implant mucositis in patients with tooth loss after surgery.Results The patients in the mucositis group had a higher proportion of diabetes and smoking,as well as a higher serum PTX3 level compared to the non-mucositis group.The PGRN level was lower than that of the non-mucositis group.The differences were statistically significant(P<0.05).Smoking,dia-betes,and PTX3 were independent risk factors for postoperative peri-implant mucositis in patients with dentition loss,while PGRN was an independent protective factor(P<0.05).The relative risk of postoperative peri-implant mucositis in dentition loss patients with PGRN ≥95.58 and PTX3 ≥ 12.64 was 0.404 times and 3.141 times that of patients with PGRN<95.58 and PTX3<12.64(P<0.05).The area under curve(AUC)of the combined prediction of PGRN and PTX3 for postoperative peri-implant mucositis in patients with dentition loss was 0.945,which was superior to the single prediction of PGRN and PTX3.Conclusion In patients with Edentulism who developed peri-implant mucositis after surgery,the serum levels of PGRN decreased while the levels of PTX3 increased.The combination of these two indicators has a higher value in predicting the occurrence of peri-implant mucositis in patients with Edentulism after surgery.
