孕中期空腹血糖与糖化血红蛋白联合检测对妊娠期糖尿病的诊断价值OA
Diagnostic value of combined detection of fasting plasma glucose and gly-cosylated hemoglobin during the second trimester of pregnancy for gesta-tional diabetes mellitus
目的 评估孕中期(24~28 周)空腹血糖(FPG)联合糖化血红蛋白(HbA1c)检测对妊娠期糖尿病(GDM)的诊断价值.方法 纳入 2018 年 9 月至 2024 年 12 月于东莞仁康医院产检的 5 400 名单胎孕妇的资料进行回顾性分析.基于国际糖尿病与妊娠研究组(IADPSG)标准及 75 g 口服葡萄糖耐量试验(OGTT),确诊 GDM 为 1 505 例(GDM组),血糖正常者3 895 例(对照组).测定并比较两组的 FPG及 HbA1c 水平.评估 FPG、HbA1c 单项及联合检测对GDM的诊断性能,包括敏感度、特异度及 ROC曲线.结果 GDM组孕妇的 FPG与 HbA1c 水平均高于对照组,差异有统计学意义(P<0.001).FPG 联合 HbA1c 诊断 GDM 的敏感度为 57.9%,特异度为 88.9%,AUC 为0.784.联合检测的 AUC 高于单项 FPG(AUC=0.767,P<0.001)和 HbA1c(AUC=0.674,P<0.001).结论 孕中期 FPG与HbA1c 联合检测对 GDM具有良好诊断效能,其综合性能(敏感度、特异度及 AUC)优于单项指标,可作为有效的辅助筛查手段,临床应用前景良好.
Objective To evaluate the diagnostic value of combining fasting plasma glucose(FPG)and glycated hemoglobin(HbA1c)measurements during the second trimester of pregnancy(24-28 weeks)for gestational diabetes mellitus(GDM).Methods A retrospective analysis was conducted on the case data of 5 400 singleton pregnant women who underwent prena-tal examination at Dongguan Renkang Hospital from September 2018 to December 2024.Based on the International Associ-ation of Diabetes and Pregnancy Study Groups(IADPSG)criteria and the 75 g oral glucose tolerance test(OGTT),1 505 women were diagnosed with GDM(GDM group),and 3 895 women had normal glucose levels(control group).FPG and HbA1c levels were measured and compared between the two groups.The diagnostic performance of FPG alone,HbA1c alone,and their combination for GDM was assessed,including sensitivity,specificity,and ROC curves.Results FPG and HbA1c levels in the GDM group were higher than those in the control group,the differences were statistically significant(P<0.001).The combination of FPG and HbA1c for GDM diagnosis yielded a sensitivity of 57.9%,a specificity of 88.9%,and an AUC of 0.784.The AUC of the combined detection was higher than that of FPG alone(AUC=0.767,P<0.001)and HbA1c alone(AUC=0.674,P<0.001).Conclusion The combined detection of FPG and HbA1c in the second trimester of pregnancy demonstrates good diagnostic efficacy for GDM.Its overall performance(sensitivity,specificity,and AUC)is superior to that of either indicator alone,suggesting its potential as an effective auxiliary screening tool with promising clinical application prospects.
