目的:探讨黄芪甲苷(AS-Ⅳ)和阿魏酸(FA)组分配伍调控糖尿病皮肤溃疡糖脂代谢对糖尿病创面愈合的影响.方法:复制2型糖尿病大鼠模型,于造模成功的糖尿病大鼠脊柱两侧的脊旁区域,环形切除直径约2 cm的全层皮肤以建立糖尿病皮肤溃疡大鼠模型(DCU),并随机分为AS-Ⅳ/FA组(DCU+50 mg/kgAS-Ⅳ/FA,AS-Ⅳ与FA质量比为10∶1)、模型组(DCU+0.9%NaCl注射液),另设空白组(0.9%NaCl注射液),均采用腹腔注射给药.采集第7、14和21天大鼠皮肤溃疡组织,检测各组创面愈合率.蛋白质印迹法(Western blotting)检测溃疡组织PI3K/Akt/GSK3β通路相关蛋白及糖脂代谢相关蛋白(GCK、PFK-1、PKM、G6pase、LDLR、SREBP-1c、GLUT4、FOXO1)表达,酶联免疫吸附试验(ELISA)检测VEGF、eNOS水平.结果:与空白组比较,模型组大鼠创面愈合率较低(P<0.05),皮肤溃疡组织中PI3K、Akt、GSK3β表达水平降低,GCK、PFK-1、PKM、G6pase、LDLR、GLUT4、FOXO1表达降低,SREBP-1c表达升高(P<0.05),VEGF、eNOS表达降低(P<0.05);与模型组比较,AS-Ⅳ/FA组大鼠创面愈合率升高(P<0.05),皮肤溃疡组织中PI3K、Akt、GSK3β表达水平升高(P<0.05),GCK、PFK-1、PKM、G6pase、LDLR、GLUT4、FOXO1表达升高(P<0.05),SREBP-1c表达降低(P<0.05),VEGF、eNOS表达升高(P<0.05).结论:黄芪甲苷和阿魏酸组分配伍可能通过调控糖尿病皮肤溃疡糖脂代谢促进糖尿病创面愈合,上述修复过程可能与激活PI3K/Akt/GSK3β信号通路有关.

Objective:To exploring the effect of astragaloside Ⅳ(AS-Ⅳ)and ferulic acid(FA)component compatibility on regulating glucose and lipid metabolism in diabetic skin ulcers and their impact on diabetic wound healing.Methods:A type 2 diabetic rat model was established.Successful model rats underwent a circular full-thickness skin excision with a diameter of 2 cm on the deep fascia on both sides of the spine to create a diabetic skin ulcer rat model(DCU).The rats were randomly divided into an AS-Ⅳ/FA group(DCU+50 mg/kg AS-Ⅳ/FA,mass ratio of AS-Ⅳ to FA 10:1)and a model group(DCU+0.9%NaCl),and a control group(0.9%NaCl)was was also established.All groups received intraperitoneal injections.Skin ulcer tissues were collected on days 7,14,and 21 to measure the wound healing rate.Western blotting was used to detect the expression of proteins related to the PI3K/Akt/GSK3β pathway and glucose and lipid metabolism(GCK,PFK-1,PKM,G6pase,LDLR,SREBP-1c,GLUT4,FOXO1).VEGF and eNOS levels were measured by enzyme linked immunosorbent assayc(ELISA).Results:Compared with the control group,the model group showed a lower wound healing rate(P<0.05),reduced expression levels of PI3K,Akt,GSK3β,GCK,PFK-1,PKM,G6pase,LDLR,GLUT4 and FOXO1,increased expression of SREBP-1c(P<0.05),and decreased expression of VEGF and eNOS(P<0.05).Compared with the model group,the AS-Ⅳ/FA group exhibited an increased wound healing rate(P<0.05),elevated expression levels of PI3K,Akt,GSK3β,GCK,PFK-1,PKM,G6pase,LDLR,GLUT4 and FOXO1,reduced expression of SREBP-1c(P<0.05),and increased expression of VEGF and eNOS(P<0.05).Conclusion:The combination of astragaloside IV and ferulic acid may promote diabetic wound healing by regulating glucose and lipid metabolism in diabetic skin ulcers,and this repair process may be related to the the activation of PI3K/Akt/GSK3β signaling pathway.
