化痰祛瘀散结方治疗痰瘀互结型甲状腺结节临床观察及对血清白细胞介素1β、白细胞介素6的影响OA
Clinical observation of Huatan Quyu Sanjie Formula in the treatment of thyroid nodules with phlegm-blood stasis and its effects on serum interleukin-1β and interleukin-6
目的 观察化痰祛瘀散结方治疗痰瘀互结型甲状腺结节(TN)的临床疗效及对血清白细胞介素 1β(IL-1β)、IL-6 水平的影响.方法 选取 2023 年 1 月至 2024 年 5 月安徽中医药大学附属铜陵中医医院内分泌科门诊62 例痰瘀互结型良性单纯性 TN 患者,按照随机数字表法分为对照组和治疗组,每组 31 例.对照组予适碘盐饮食、适当锻炼等基础治疗,治疗组在对照组基础上加化痰祛瘀散结方治疗,疗程均为 3 个月.比较 2 组临床疗效及治疗前后中医证候积分、甲状腺功能、抗甲状腺过氧化物酶抗体(TPOAb)、抗甲状腺球蛋白抗体(TGAb)、IL-1β、IL-6水平、TN 数目、TN 最大直径.结果 治疗组总有效率 77.42%(24/31),对照组总有效率 29.03%(9/31),治疗组总有效率高于对照组(P<0.05).治疗后,治疗组中医证候积分、抗 TPOAb、抗 TGAb、血清 IL-1β、IL-6 水平及 TN 最大直径、数目均较本组治疗前及对照组治疗后降低(P<0.05).2 组患者治疗前后游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)水平组间及组内比较差异均无统计学意义(P>0.05).结论 化痰祛瘀散结方能减轻痰瘀互结型 TN 患者临床症状,减少中医证候积分,缩小 TN 直径,减少 TN 数目,降低血清抗 TPOAb、抗 TGAb、IL-1β、IL-6 水平.
Objective To observe the clinical efficacy of Huatan Quyu Sanjie Formula in treating thyroid nodules(TN)with phlegm-blood stasis and its effects on serum interleukin-1β(IL-1β)and interleukin-6(IL-6)levels.Methods A total of 62 patients with benign simple TN with phlegm-blood stasis who attended the Endocrinology Outpatient Department of Tongling Traditional Chinese Medicine Hospital Affiliated to Anhui University of Chinese Medicine from January 2023 to May 2024 were selected and randomly assigned,using a random number table,to a control group and a treatment group,with 31 patients in each group.The control group received basic treatment,including an iodized-salt diet and appropriate exercise.The treatment group received Huatan Quyu Sanjie Formula in addition to the treatment given to the control group.The treatment duration was 3 months for both groups.Clinical efficacy,traditional Chinese medicine syndrome scores,thyroid function,anti-thyroid peroxidase antibody(TPOAb),anti-thyroglobulin antibody(TGAb),IL-1β,IL-6 levels,TN number,and maximum TN diameter were compared before and after treatment in the two groups.Results The total effective rate was 77.42%(24/31)in the treatment group and 29.03%(9/31)in the control group(P<0.05),with a significantly higher total effective rate in the treatment group(P<0.05).After treatment,the treatment group showed lower traditional Chinese medicine syndrome scores,anti-TPOAb,anti-TGAb,serum IL-1β,and IL-6 levels,as well as smaller maximum TN diameter and fewer TNs,compared with both its pre-treatment values and the post-treatment values of the control group(P<0.05).There were no statistically significant between-group or within-group differences in free triiodothyronine(FT3),free thyroxine(FT4),or thyroid-stimulating hormone(TSH)levels before or after treatment in either group(P>0.05).Conclusion Huatan Quyu Sanjie Formula can alleviate clinical symptoms in patients with TN with phlegm-blood stasis,reduce traditional Chinese medicine syndrome scores,decrease TN diameter and number,and lower serum anti-TPOAb,anti-TGAb,IL-1β,and IL-6 levels.
