补中益气汤对便秘型肠易激综合征脾胃虚弱证肠道菌群的影响OA
Effects of Buzhong Yiqi Decoction on gut microbiota in patients with constipation-predominant irritable bowel syndrome of spleen-stomach deficiency pattern
目的 探究补中益气汤治疗便秘型肠易激综合征(IBS-C)脾胃虚弱证的临床效果及对肠道菌群的影响.方法 将 120 例 IBS-C 患者采用随机数字表法分为治疗组和对照 A 组,每组60 例.另选60 例性别、年龄匹配的同期体检健康者为对照 B 组.对照 A 组予西医常规治疗,治疗组予补中益气汤治疗.比较治疗组与对照 A 组中医证候积分(TCMSS)、克里夫兰便秘评分量表(CCS)、IBS 生活质量量表(IBS-QOL)评分,生长抑素(SS)、神经肽 Y(NPY)、P 物质(SP)、肿瘤坏死因子 α(TNF-α)、白细胞介素 6(IL-6)、IL-8 水平,模拟排便动作下肛管直肠动力学指标最大肛门收缩压(MSP)、肛门括约肌净减压、直肠感知阈值(RVST)、排便阈值、直肠顺应性(RC),肠道菌群数量,临床疗效、不良反应及复发情况.采用 Hiseq2000 系统进行高通量测序,分析菌群相对丰度.通过 Alpha 多样性指数分析来评估肠道菌群的多样性特征,然后进行主成分分析.结果 治疗后,与对照 A 组相比,治疗组 TCMSS 评分、CCS、肛门括约肌净减压、RVST、排便阈值、RC、SS、NPY、TNF-α、IL-6、IL-8 水平及肠杆菌、肠球菌、拟杆菌数量均更低(P<0.05),IBS-QOL 评分、MSP、SP 水平及双歧杆菌、乳酸杆菌数量均更高(P<0.05).治疗组总有效率 93.33%(56/60),对照组总有效率80.00%(48/60),治疗组总有效率高于对照 A 组(P<0.05).2 组不良反应发生率比较差异无统计学意义(P>0.05).与对照 A组相比,治疗组复发率更低(P<0.05).治疗组肠道菌群相对丰度和多样性均显著优于对照 A 组.治疗后,与对照 A 组相比,治疗组与对照B 组在二维坐标系中的位置更接近.结论 补中益气汤能明显改善IBS-C 患者临床症状,且对肠道菌群相对丰度和多样性也有明显改善,接近于体检健康者.
Objective To investigate the clinical efficacy of Buzhong Yiqi Decoction in treating constipation-predominant irritable bowel syndrome(IBS-C)with spleen-stomach deficiency pattern,and its effects on gut microbiota.Methods A total of 120 patients with IBS-C were randomly assigned,using a random number table,to a treatment group and a control A group,with 60 patients in each group.In addition,60 age-and sex-matched healthy individuals undergoing physical examination during the same period were selected as control B group.The control A group received conventional Western medical treatment,while the treatment group received Buzhong Yiqi Decoction.The two patient groups were compared in terms of traditional Chinese medicine syndrome score(TCMSS),Cleveland Clinic Constipation Score(CCS),Irritable Bowel Syndrome-Quality of Life questionnaire(IBS-QOL)score,levels of somatostatin(SS),neuropeptide Y(NPY),substance P(SP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and IL-8,anorectal dynamic parameters during simulated defecation,including maximal anal squeeze pressure(MSP),net anal sphincter decompression,rectal volume at sensory threshold(RVST),defecation threshold,and rectal compliance(RC),gut microbiota counts,clinical efficacy,adverse reactions,and recurrence.High-throughput sequencing was performed using the Hiseq2000 system to analyze the relative abundance of the microbiota.Alpha diversity indices were used to assess the diversity of the gut microbiota,followed by principal component analysis.Results After treatment,compared with the control A group,the treatment group showed lower TCMSS,CCS,net anal sphincter decompression,RVST,defecation threshold,RC,SS,NPY,TNF-α,IL-6,and IL-8 levels,as well as lower counts of Enterobacter,Enterococcus,and Bacteroides(P<0.05).The treatment group showed higher IBS-QOL scores,MSP,SP levels,and counts of Bifidobacterium and Lactobacillus(P<0.05).The total effective rate was 93.33%(56/60)in the treatment group and 80.00%(48/60)in the control group,with a significantly higher total effective rate in the treatment group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Compared with the control A group,the recurrence rate was lower in the treatment group(P<0.05).The relative abundance and diversity of the gut microbiota in the treatment group were significantly better than those in the control A group.After treatment,compared with the control A group,the positions of the treatment group and control B group were closer in the two-dimensional coordinate system.Conclusion Buzhong Yiqi Decoction can significantly improve clinical symptoms in patients with IBS-C and markedly improve the relative abundance and diversity of the gut microbiota,bringing them closer to those of healthy individuals undergoing physical examination.
