Th17/Treg及IL-6在不同分期胃癌表达水平及其对预后的影响OA
Expression Levels of Th17/Treg and IL-6 level in Gastric Cancer at Different Stages and Their Impact on Prognosis
目的:探究 Th17/Treg 及 IL-6 在不同分期胃癌表达水平及其对预后的影响.方法:2021年1 月至2024 年1 月我院收治的胃癌患者共110 例,将患者按照 TNM 分期分为Ⅰ期(42 例)、Ⅱ期(28例)、Ⅲ期(26 例)以及Ⅳ期(14 例),分析各组一般资料及 Th17、Treg、Th17/Treg(入组后次日采集空腹静脉血5mL,采用流式细胞仪检测)及 IL-6 水平,采用 Pearson 分析相关性.随访 1 年按照预后情况分为预后良好组(n=84)及预后不良组(n=26),比较 Th17、Treg、Th17/Treg 及 IL-6 水平,采用 ROC 评估Th17、Treg、Th17/Treg 及 IL-6 对预后的诊断价值.结果:与Ⅰ期相比,Ⅱ期肿瘤直径均升高(P<0.05),而在分化程度、浸润深度、淋巴结转移比较无统计学意义(P>0.05);与Ⅱ期相比,Ⅲ期肿瘤直径、低分化占比、浸润深度在黏膜层以下占比、淋巴结转移均升高(P<0.05);与Ⅲ期比较,Ⅳ期肿瘤直径、分化程度、浸润深度、淋巴结转移比较无统计学意义(P>0.05);随着患者 TNM 分期级别越高,患者 Th17、Treg、Th17/Treg 及 IL-6 水平升高越明显,且有统计学差异(P<0.05);肿瘤分期与 Th17、Treg、Th17/Treg 及IL-6 均呈现正相关(r=0.902、0.738、0.813 及 0.654,P<0.001).Ⅰ期患者生存率为 92.86%,Ⅱ期患者生存率82.14%,Ⅲ期患者生存率65.38%,Ⅳ期患者生存率 35.71%.随着患者 TNM 分期级别越高,患者生存率降低越明显,且有统计学差异(P<0.05).与预后良好组相比,预后不良组的 Th17、Treg、Th17/Treg 及 IL-6 水平显著升高(P<0.05);对胃癌患者预后的诊断 AUC 面积从高到低分别为 Th17/Treg、Th17、Treg、IL-6,且联合 AUC 面积最高为0.921.结论:随着胃癌分期进展,Th17、Treg、Th17/Treg及 IL-6 水平显著升高,且与患者预后不良密切相关,联合检测对胃癌预后具有较高诊断价值.
Objective:To investigate the expression levels of Th17/Treg and IL-6 level in gastric cancer(GC)at different stages and their impact on prognosis.Methods:A total of 110 patients with gastric cancer admitted to our hospital from January 2021 to January 2024 were enrolled and divided into Stage Ⅰ(n=42),Stage Ⅱ(n=28),Stage Ⅲ(n=26)and Stage Ⅳ(n=14)according to TNM staging.General clinical da-ta,the levels of Th17,Treg,Th17/Treg ratio and IL-6 were analyzed among all groups.Fasting venous blood(5 mL)was collected on the next day after enrollment,and Th17,Treg were detected by flow cytometry,with IL-6 level measured simultaneously.Pearson correlation analysis was used to evaluate the correlation between indicators and TNM staging.All patients were followed up for 1 year and further divided into a favorable prog-nosis group(n=84)and a poor prognosis group(n=26)based on prognostic outcomes.The levels of Th17,Treg,Th17/Treg ratio and IL-6 level were compared between the two prognostic groups,and receiver operat-ing characteristic(ROC)curve analysis was performed to assess the diagnostic value of these indicators for the prognosis of gastric cancer patients.Results:Compared with Stage Ⅰ,the tumor diameter in Stage Ⅱ was sig-nificantly increased(P<0.05),while no significant differences were observed in differentiation degree,infil-tration depth and lymph node metastasis(P>0.05).Compared with Stage Ⅱ,Stage Ⅲ had significantly lar-ger tumor diameter,higher proportion of poorly differentiated tumors,higher proportion of tumors with infiltra-tion depth below the mucosal layer and higher incidence of lymph node metastasis(P<0.05);no significant differences were observed in the above indicators between Stage Ⅳ and Stage Ⅲ(P>0.05).The levels of Th17,Treg,Th17/Treg ratio and IL-6 level increased significantly with the elevation of TNM staging,with statistically significant differences(P<0.05).TNM stage was positively correlated with the levels of Th17,Treg,Th17/Treg ratio and IL-6 level(r=0.902,0.738,0.813 and 0.654,respectively,all P<0.001).The 1-year survival rates were 92.86%for Stage Ⅰ,82.14%for Stage Ⅱ,65.38%for Stage Ⅲ and 35.71%for Stage Ⅳ patients,and decreased significantly with the progression of TNM stage(P<0.05).The lev-els of Th17,Treg,Th17/Treg ratio and IL-6 level in the poor-prognosis group were significantly higher than those in the favorable-prognosis group(P<0.05).For the prognostic diagnosis of gastric cancer,the area un-der the ROC curves(AUCs)was ranked from high to low:Th17/Treg ratio,Th17,Treg and IL-6 level,and the combined detection of these indicators yielded the highest AUC of 0.921.Conclusion:With the pro-gression of TNM stage,the levels of Th17,Treg,Th17/Treg ratio and IL-6 level significantly increase,which are closely associated with poor prognosis and combined detection of these markers demonstrates high diagnos-tic value for predicting GC prognosis.
