Evidence mapping of ICU diary interventions for critically ill patients

目的:加强监护病房(intensive care unit,ICU)日记是指医务人员及患者家属在患者入住ICU期间,通过文字或图片记录患者在ICU治疗期间发生的重要事件.ICU日记被认为能有效改善患者预后及生活质量.然而,ICU日记在记录形式及交付时间等方面的研究结论并不一致.本研究旨在通过证据图谱系统描述和评价ICU日记干预的相关研究,为临床实践及指南制订提供参考.方法:检索国内外相关数据库,检索时限为建库至2026年4月1日,选择重症患者ICU日记干预的随机对照试验(randomized controlled trials,RCTs)及系统评价/荟萃分析.采用Cochrane手册5.1.0推荐的偏倚风险评估工具及评估系统评价的测量工具2(A Measurement Tool to Assess Systematic Reviews 2,AMSTAR-2)对纳入的研究进行质量评价,利用Microsoft Excel 2019进行数据提取与编码,采用气泡图综合呈现纳入的研究内容.结果:共纳入12篇RCTs和9篇系统评价/荟萃分析.干预效果方面,10篇RCTs和9篇系统评价/荟萃分析的结论为"有益"或"可能有益".交付时间方面,RCTs中涉及2种(ICU住院期间和ICU转出后);系统评价/荟萃分析中的交付时间不统一.记录形式方面,RCTs中涉及5种,即"护士书写""家属书写""医生和护士共同书写""医生、护士和家属共同书写"及"研究小组书写";系统评价/荟萃分析中涉及3种,即"医生、护士和家属共同书写""医生、护士、家属和研究小组共同书写"和"医生、护士、家属、研究小组和其他相关人员共同书写".结局指标方面,RCTs及系统评价/荟萃分析中均包含心理社会健康、生理健康、感知的健康及功能健康等领域.结论:ICU日记对改善重症患者的结局有一定潜在积极效果,但其实施与管理在记录人员、撰写内容、交付时间及干预效果等方面有待进一步探索,建议未来开展高质量深入研究并完善相关管理规范.

Objective:Intensive care unit(ICU)diaries refer to written or pictorial records created by healthcare professionals and family members of patients during ICU hospitalization to document important events occurring throughout the patient's treatment.ICU diaries are considered an effective intervention for improving patient outcomes and quality of life.However,existing studies have reported inconsistent findings regarding diary formats and delivery timing.This study aims to systematically describe and evaluate the evidence on ICU diary interventions through evidence mapping,thereby providing references for clinical practice and guideline development. Methods:Relevant Chinese and international databases were searched from inception to April 1,2026.Randomized controlled trials(RCTs)and systematic reviews/Meta-analyses investigating ICU diary interventions in critically ill patients were included.The risk of bias in RCTs was assessed using the Cochrane Handbook 5.1.0 risk-of-bias assessment tool,while the methodological quality of systematic reviews was evaluated using A Measurement Tool to Assess Systematic Reviews 2(AMSTAR-2).Data extraction and coding were conducted using Microsoft Excel 2019,and bubble plots were generated to comprehensively present the characteristics of the included studies. Results:A total of 12 RCTs and 9 systematic reviews/Meta-analyses were included.Regarding intervention effectiveness,10 RCTs and all 9 systematic reviews/Meta-analyses concluded that ICU diary interventions were"beneficial"or"possibly beneficial".Regarding delivery timing,2 approaches were identified in the RCTs,namely delivery during ICU hospitalization and after ICU discharge,whereas delivery timing varied across systematic reviews/Meta-analyses.Regarding diary format,5 recording approaches were identified in the RCTs,including nurse-written diaries,family-written diaries,jointly written diaries by physicians and nurses,jointly written diaries by physicians,nurses,and family members,and diaries written by research teams.Three recording approaches were identified in systematic reviews/Meta-analyses,including jointly written diaries by physicians,nurses,and family members;by physicians,nurses,family members,and research teams;and by physicians,nurses,family members,research teams,and other relevant personnel.Outcome indicators in both RCTs and systematic reviews/Meta-analyses covered psychosocial health,physical health,perceived health,and functional health. Conclusion:ICU diary interventions may have potential positive effects on improving outcomes in critically ill patients.Further exploration is needed regarding implementation and management strategies,including diary authorship,content development,delivery timing,and outcome evaluation.Future high-quality studies are warranted to strengthen the evidence base and establish standardized management protocols.
