目的:分析早产儿桡动脉采血后发生动脉痉挛的关键诱因,并构建系统化护理干预方案.方法:回顾性分析 6例早产儿桡动脉采血后并发动脉痉挛的临床资料,从患儿、操作者、环境与时机及血管特性四个维度进行诱因分析,制定并实施"采血前-采血中—痉挛发生后"三阶段护理策略.结果:干预后6 例患儿动脉痉挛症状均在 1 小时内缓解.主要诱因为体温管理不足(<36.5℃)、反复穿刺(>2次)及按压力度过大(>50 mmHg).结论:针对早产儿桡动脉采血后动脉痉挛,实施涵盖采血前风险预警、采血中精准操作、痉挛发生后即刻响应与阶梯干预的全流程管理策略,可有效缓解痉挛症状,降低动脉痉挛发生率,改善患儿临床结局,具有临床推广价值.

Objective:To analyze the key predisposing factors for arterial spasm following radial artery blood collection in preterm in-fants and to develop a systematic nursing optimization strategies.Methods:A retrospective analysis was conducted on the clinical data of six preterm infants who developed arterial spasm following radial artery blood collection.The predisposing factors were analyzed from four dimen-sions:infant-related factors,operator-related factors,environmental and temporal factors,and vascular characteristics.A three-stage nursing strategy was formulated and implemented:precollection,during sampling,and post-spasm.Results:After intervention,arterial spasm in all six cases was relieved within 1 hour.The main predisposing factors were inadequate thermoregulation(body temperature<36.5℃),repeated punc-tures(>2 attempts),and excessive compression pressure(>50 mmHg).Conclusions:For arterial spasm after radial artery blood collection in preterm infants,implementing a full-process management strategy encompassing pre-collection risk warning,precise operation during sam-pling,and immediate response with stepwise intervention after spasm can effectively relieve spasm symptoms,reduce the incidence of arterial spasm,and improve clinical outcomes.This strategy is worthy of clinical promotion.
