基于FAERS和JADER数据库的达格列净治疗心力衰竭不良事件挖掘OA
Data mining of adverse events of dapagliflozin in the treatment of heart failure based on FAERS and JADER databases
目的:通过挖掘 FAERS 和 JADER数据库心力衰竭患者达格列净的阳性信号,探讨该药物潜在的不良事件(ADE),为临床安全合理用药提供参考.方法:收集FAERS和JADER中2014年第一季度到2025年第一季度的达格列净的ADE报告,采用ROR、MHRA、BCPNN与MGPS法4种方法对信号进行分析.结果:共提取到达格列净ADE报告2162份(FAER 1805份,JADER 357份),经过多重筛查,挖掘出81个PT信号,涉及15个SOC,感染及侵染性疾病所包含的PT信号最多(21个,25.93%).结论:心力衰竭患者达格列净ADE主要集中在代谢及营养类疾病、感染及侵染类疾病和全身性疾病及给药部位的各种反应等方面.信号强度较高的骨炎、红细胞增多症、包茎、全身性念珠菌病等未被药品说明书记载,临床实践应警惕相关ADE.
Objective:This study leverages pharmacovigilance data from the FDA Adverse Event Reporting System(FAERS)and the Japanese Adverse Drug Event Report(JADER)databases to identify and characterize potential adverse drug events(ADEs)associated with da-pagliflozin in patients with heart failure,thereby informing clinical risk assessment and safe prescribing practices.Methods:All dapagliflozin-related ADE reports submitted between Q1 2014 and Q1 2025 were retrieved from FAERS and JADER.Signal detection was performed using four complementary quantitative methods-Reporting Odds Ratio(ROR),the UK Medicines and Healthcare products Regulatory Agency(MH-RA)method,Bayesian Confidence Propagation Neural Network(BCPNN),and Multi-item Gamma Poisson Shrinker(MGPS).Results:A total of 2,162 dapagliflozin-associated ADE reports were identified(FAERS:n=1,805;JADER:n=357).Following multi-layered signal validation,81 preferred term(PT)signals were confirmed,spanning 15 System Organ Classes(SOCs).Infections and infestations constituted the largest SOC category,contributing 21 PTs(25.9%).Conclusions:In heart failure populations,dapagliflozin-associated ADEs are disproportionately represented in metabolic and nutritional disorders,infections and infestations,and general disorders and administration site conditions.The identifi-cation of several novel,label-unsupported high-strength signals-including osteomyelitis(not"osteitis"),polycythemia,phimosis,and systemic candidiasis-underscores the need for heightened clinical vigilance against these associated advers drug events in routine practice.
