目的:比较个体化 Fraxion 体位固定技术与塑形垫联合热塑膜体位固定技术在肺癌脑转移瘤立体定向放射治疗中的精度差异.方法:选取2024年1~12月我院放疗科收治的符合立体定向头部放疗适应证的肺癌脑转移患者39例,随机分为两组,19例采用个体化Fraxion体位固定,20例采用塑形垫联合热塑膜固定.借助Elekta-infinity医用直线加速器采集两组摆位误差数据,并开展统计学分析.结果:两种固定方法在三维平移误差与旋转误差上,差异均无统计学意义(P>0.05);但 Fraxion 组摆位误差整体更小,其绝对平移误差、旋转误差均低于塑形垫组,且显著减小了靶区 Y 方向的外放边界.结论:个体化 Fraxion体位固定技术与塑形垫联合热塑膜固定技术,均能满足肺癌脑转移瘤SRT治疗的靶区精度与重复性要求,具备临床应用价值.其中Fraxion体位固定技术在多维度平移误差、旋转误差控制上更具优势,可缩小靶区Y方向外放边界,为治疗精度提升提供更多支持.

Objective:This study aimed to compare the precision differences between the individualized Fraxion patient positioning and fixation technique and the positioning and fixation technique combining a shaping pad with a thermoplastic film in stereotactic radiotherapy for brain metastases from lung cancer.Methods:A total of 39 patients with lung cancer brain metastases who met the indications for stereotactic cranial radiotherapy and were admitted to the Department of Radiation Oncology,Gansu Provincial Cancer Hospital from January to December 2024 were selected.The patients were randomly divided into two groups.Nineteen patients were fixed using the individualized Fraxion position-ing technique,while the remaining 20 patients were fixed with the combination of a shaping pad and a thermoplastic film.Two sets of setup error data were collected using an Elekta-infinity medical linear accelerator,and statistical analysis was conducted.Results:There were no statisti-cally significant differences in the three-dimensional translational errors and rotational errors between the two fixation methods.However,the overall setup errors in the Fraxion group were smaller:both the absolute translational errors and rotational errors were lower than those in the shaping pad group,and the expansion margin of the target volume in the Y-axis direction was significantly reduced.Conclusions:Both the in-dividualized Fraxion positioning and fixation technique and the shaping pad combined with thermoplastic film fixation technique can meet the requirements for target volume precision and repeatability in SRT for brain metastases from lung cancer,and thus have clinical application val-ue.Among them,the Fraxion positioning and fixation technique has greater advantages in controlling multi-dimensional translational errors and rotational errors,and can reduce the expansion margin of the target volume in the Y-axis direction,providing more support for improving treat-ment precision.
