基于脾主四肢肌肉探讨黄芪桂枝五物汤治疗肢体麻木OA

Limb numbness is a common clinical symptom,which falls into the categories of"blood bi syndrome"and"numbness and insensitivity"in traditional Chinese medicine(TCM).The core pathogenesis of limb numbness includes impaired transportation due to spleen deficiency,insufficiency of qi and blood,disharmony between nutrient and defense qi,and blockage of channels and collaterals.Huangqi Guizhi Wuwu decoction is a representative formula from Synopsis of Golden Chamber for the treatment of blood bi syndrome,and it has a beneficial effect on limb numbness caused by various etiological factors in clinical practice.This study adopted a combination of theoretical analysis and clinical case observation to analyze the pathological correlation between limb numbness and spleen-qi deficiency based on the theory that"the spleen governs the four limbs and muscles".It explored the relationship between Huangqi Guizhi Wuwu decoction and the etiology and pathogenesis of limb numbness according to the formulation principles.In Huangqi Guizhi Wuwu decoction,Radix Astragali seu Hedysari serves as the sovereign medicinal to invigorate the spleen and replenish qi.Combined with Ramulus Cinnamomi,Radix Paeoniae Albaand other ingredients,it exerted a synergistic effect of reinforcing qi,unblocking yang and harmoning nutrient qi,thereby acting precisely on the pathogenesis of bi syndrome induced by spleen deficiency.This study clarified the theoretical basis and clinical efficacy of Huangqi Guizhi Wuwu decoction in the treatment of limb numbness,and corroborated the clinical value of the theory that"the spleen governs the four limbs and muscles".It is of great significance for promoting the modern application of classic famous prescriptions and optimizing the standardized TCM treatment regimen for limb numbness.
