半夏泻心汤对Hp阳性胃溃疡的临床效应及对Ghrelin、PYY的影响研究OA
Study on the Clinical Effect of Banxia Xiexin Decoction on Hp-positive Gastric Ulcer and Its Influence on Ghrelin and PYY
目的 观察半夏泻心汤对幽门螺杆菌(Hp)阳性胃溃疡的临床效应及对胃促生长素(Ghrelin)及酪酪肽(PYY)的影响.方法 选取南阳张仲景医院门诊2024年3月—2025年3月收治的Hp阳性胃溃疡患者90例,随机分为2组,每组45例.对照组予常规治疗,观察组在对照组的基础上加用半夏泻心汤口服,对比2组疗效、Ghrelin及PYY水平.结果 治疗后,观察组中医证候积分显著低于对照组(P<0.05);观察组溃疡愈合率、总有效率均高于对照组,差异有统计学意义(P<0.05);观察组Hp根除率高于对照组(P<0.05);治疗后,观察组Ghrelin水平低于对照组,PYY水平高于对照组,差异均有统计学意义(P<0.05);观察组总有效率高于对照组(P<0.05).结论 半夏泻心汤联合常规治疗能够有效减轻Hp阳性胃溃疡患者临床症状,提高溃疡愈合率、Hp清除率,继而提升临床疗效,其机制可能与半夏泻心汤能调节胃肠激素平衡有关.
Objective To observe the clinical effect of Banxia Xiexin decoction on helicobacter pylori(Hp)-positive gastric ulcer and its influence on Ghrelin and Peptide YY(PYY).Methods A total of 90 patients with Hp-positive gastric ulcer admitted to the outpatient clinic of Nanyang Zhang Zhongjing Hospital from March 2024 to March 2025 were selected and randomly divided into two groups,with 45 cases in each group.The control group received conventional treatment,while the observation group was treated with Banxia Xiexin decoction orally in addition to the conventional treatment.The efficacy,levels of Ghrelin and PYY were compared between the two groups.Results After treatment,the traditional Chinese medicine syndrome score of the observation group was significantly lower than that of the control group(P<0.05).The ulcer healing rate and total effective rate of the observation group were higher than those of the control group,with statistically significant differences(P<0.05).The Hp eradication rate of the observation group was higher than that of the control group(P<0.05).After treatment,the Ghrelin level of the observation group was lower,while the PYY level was higher than that of the control group,with statistically significant differences(P<0.05).The total effective rate of the observation group was higher than that of the control group(P<0.05).Conclusion Banxia Xiexin decoction combined with conventional treatment could effectively alleviate the clinical symptoms of patients with Hp-positive gastric ulcer,improve the ulcer healing rate and Hp eradication rate,and thus enhance the clinical efficacy.The mechanism might be related to the regulation of gastrointestinal hormone balance by Banxia Xiexin decoction.
