胆胰肠结合部疾病规范化诊治的思考与探讨OA

The pancreaticobiliary junction is a valve that controls the excretion of bile and pancreatic juice.It has been described as a"hub project that combines the biliary tract basin and the pancreatic duct basin"by Academician Huang Zhiqiang.The pancreaticobiliary junction diseases may induce various benign and malignant diseases of the biliary and pancreatic systems,such as common bile duct stones,pancreatitis and gallbladder cancer,by altering the flow rate and direction of bile and pancreatic juice.However,the anatomical location of the pancreaticobiliary junction is special,and the pancreaticobiliary junction diseases are insidious onset and lack of characteristic clinical manifestations,which lead to insufficient understanding by clinicians and causes missed diagnosis,misdiagnosis,and over-treatment.Inder to enhance clinicians'understanding of the pancreaticobiliary junction diseases and standardize the diagnosis and treatment of such conditions,the Gallbladder-Preserving Surgery Committee,Endoscopy Specialist Branch of Chinese Medical Doctor Association and Expert Consensus Collaborative Group on Endoscopic Diagnosis and Treatment of the Benign Pancreaticobiliary Junction Disease published"Expert consensus on endoscopic diagnosis and treatment of the benign pancreaticobiliary junction disease(2023 edition)"(published in Chinese Medical Journal,2023,Volume 103,Issue 40).Branch of Biliary Surgery,Chinese Society of Surgery,Chinese Medical Association and Biliary Surgery Expert Working Group,Society of Surgeons,Chinese Medical Doctor Association published"Consensus on standardized diagnosis and treatment of diseases at the choledocho-pancreatico-duodenal junction(2026)"(published in Chinese Journal of Surgery,Volume 64,Issue 3).The author of this article has benefited a lot from the above consensus,but some of the expressions and opinions are questionable.Based on the author's work experience and relevant literature,several thoughts and discussions are put forward regarding the content of the consensus.Any inappropriate points are sincerely hoped to be corrected.
