减重手术对男性勃起功能及精液质量的影响OA

Objective:To investigate the improvement effect of laparoscopic sleeve gastrectomy(LSG)on erectile function,se-men quality and related metabolic indicators in male obese patients complicated with erectile dysfunction(ED).Methods:A retrospec-tive cohort study was conducted to analyze the clinical data of 61 male patients with obesity combined with ED who underwent LSG from Jan.2020 to Jan.2024.The BMI,international index of erectile function-5 score,semen parameters,serum homocysteine and fibroblast growth factor-21 were compared before operation,6 months,and 12 months after operation.Results:After surgery,the BMI decreased from(38.0±5.2)kg/m2 before operation to(29.2±3.3)kg/m2;glycated hemoglobin decreased from(6.94%±0.39%)to(5.42%±0.38%).Other indicators including waist-to-hip ratio,triglycerides,total cholesterol,low-density lipoprotein,serum homocysteine and fibroblast growth factor-21 were significantly improved(P<0.001).The international index of erectile function-5 score increased from(8.3±2.6)points(moderate erectile dysfunction)to(18.2±2.3)points(mild erectile dysfunction).The proportion of progressively motile sperm increased from(18.8%±2.8%)to(46.7%±4.4%)with significant difference(P<0.001).Conclusions:LSG can effec-tively reduce body weight,improve glucose and lipid metabolism,restore erectile function and enhance semen quality.It is an effective intervention method to improve obesity-related male reproductive health problems.
