耳内镜下岛状耳屏软骨-软骨膜在Ⅱ型鼓室成形术中的应用及疗效分析OA
Application and efficacy analysis of island shaped tragal cartilage perichondrium under otoendoscopy in type Ⅱ tympanoplasty
目的 探讨耳内镜下岛状耳屏软骨-软骨膜在Ⅱ型鼓室成形术中的应用及疗效.方法 回顾性分析宁波市医疗中心李惠利医院收治的93例(95耳)慢性化脓性中耳炎患者,均在耳内镜下采用岛状耳屏软骨-软骨膜复合体夹层法行Ⅱ型鼓室成形术,观察患者术后并发症,鼓膜愈合率及术后3个月听力情况.结果 95耳术后共愈合92例(96.8%).气导听力相较术前显著提高者90例(94.7%).比较术前术后骨导差异及不同长度听小骨术后气骨导差,差异无统计学意义(P>0.05).术后气导及气骨导差相较术前显著缩小,差异有统计学意义(P<0.01).术后均未出现并发症.结论 耳内镜下采用岛状耳屏软骨-软骨膜复合体夹层法行Ⅱ型鼓室成形术可获得较高的鼓膜愈合率并可显著提高患者听力.
OBJECTIVE To explore the application and clinical efficacy of island tragus cartilage-perichondrium under otoscope in type Ⅱ tympanoplasty.METHODS A retrospective analysis was performed on 93 patients(95 ears)with chronic suppurative otitis media treated at Ningbo Medical Center Lihuili Hospital.All patients underwent type Ⅱtympanoplasty using the island tragus cartilage-perichondrium complex interlay method under endoscopy.Postoperative complications,tympanic membrane healing rate,and hearing status at 3 months after surgery were observed.RESULTS Of the 95 ears,92 cases(96.8%)achieved postoperative healing.Air conduction hearing was significantly improved in 90 cases(94.7%)compared with preoperative levels.There were no statistically significant differences in bone conduction between pre and post operation,nor in postoperative air bone gap among patients with different ossicle lengths(P>0.05).Postoperative air conduction hearing and air bone gap were significantly improved compared with preoperative values,with statistically significant differences(P<0.01).No postoperative complications occurred.CONCLUSION Type Ⅱ tympanoplasty using the island tragus cartilage-perichondrium complex interlay method under otologic endoscopy can achieve a high tympanic membrane healing rate and significantly improve hearing in patients.
