目的 通过16S rRNA基因测序分析不同分期CKD患者肠道菌群代谢特征并筛选疾病进展相关微生物标志物.方法 纳入2022年11月至2023年10月于该院肾内科就诊的148例受试者,包括54例健康对照者(HC组)、67例 CKD 1期患者(GFR-NOR组),27例 CKD 2~3期患者(GFR-DIS组).通过16S rRNA基因测序分析肠道菌群结构与功能.结果 在门分类水平,梭杆菌门(Fusobacteri)的丰度在 HC组、GFR-NOR组、GFR-DIS组中逐渐增加,绿弯菌门(Chloroflexi)、放线菌门(Actinobacteria)等的丰度逐渐降低.GFR-NOR组(8.26%)和 GFR-DIS 组(10.40%)的拟杆菌门(Bacteroidetes)平均丰度高于 HC 组(7.94%);GFR-NOR 组(19.34%)和 GFR-DIS 组(18.62%)的 Actinobacteria 平均丰度低于 HC 组(23.71%).与 HC组比较,GFR-NOR组和GFR-DIS组肠道微生物的α多样性降 低,Shannon指数和Goods_coverage指数在3组间差异有统计学意义(P<0.05).3组间的肠道微生物β多样性中,HC组与 GFR-NOR组、HC组与GFR-DIS组主成分分析(PCA)和主坐标分析(PCoA)在第一轴上的分布差异均有统计学意义(P<0.05).肠杆菌科(Enterobacteriaceae)的丰度与eGFR呈负相关(P<0.05),与收缩压、舒张压、LDL-C等呈正相关(P<0.05);克雷伯菌属(Klebsiella)的丰度与收缩压、舒张压、LDL-C、血肌酐、血糖呈负相关(P<0.05),与 HDL-C呈正相关(P<0.05).结论 早期CKD即存在特异性菌群失调,肠道菌群或可作为CKD诊断与治疗的新型靶点.

Objective To analyze the metabolic characteristics of the gut microbiota in patients with dif-ferent stages of CKD,and to screen microbial markers related to disease progression through 16S rRNA gene sequencing.Methods A total of 148 subjects who visited the Nephrology Department of Beijing Chaoyang Hospital,Capital Medical University from November 2022 to October 2023 were included,including 54 healthy controls(HC group),67 patients with CKD stage 1(GFR-NOR group),and 27 patients with CKD stages 2-3(GFR-DIS group).The structure and function of the gut microbiota were analyzed through 16S rRNA gene sequencing.Results At the phylum level,the abundance of Fusobacteri gradually increased in the HC group,GFR-NOR group,and GFR-DIS group,while the abundances of Chloroflexi,Actinobacteria,etc.gradually de-creased.The average relative abundance of the Bacteroidetes phylum in the GFR-NOR group(8.26%)and the GFR-DIS group(10.40%)was higher than that in the HC group(7.94%);the average relative abundance of the Actinobacteria phylum in the GFR-NOR group(19.34%)and the GFR-DIS group(18.62%)was low-er than that in the HC group(23.71%).Compared with the HC group,the α diversity of the gut microbiota in the GFR-NOR group and the GFR-DIS group decreased,and the Shannon index and Goods_coverage index showed statistically significant differences among the three groups(P<0.05).In the gut microbiota β diversi-ty among the three groups,there were statistically significant differences in the distribution on the first axis in both PCA and PCoA between the HC group and the GFR-NOR group,and between the HC group and the GFR-DIS group(P<0.05).The abundance of Enterobacteriaceae was negatively correlated with eGFR(P<0.05),and positively correlated with systolic blood pressure,diastolic blood pressure,LDL-C,serum creati-nine,and blood glucose(P<0.05);the abundance of Klebsiella was negatively correlated with systolic blood pressure,diastolic blood pressure,LDL-C,serum creatinine,and blood glucose(P<0.05),and positively cor-related with HDL-C(P<0.05).Conclusion In the early stage of CKD,there is already a specific dysbiosis of the gut microbiota.The gut microbiota may serve as a new target for the diagnosis and treatment of CKD.
