新型冠状病毒、流感病毒和腺病毒肺炎的临床特征的比较OA
Comparison of clinical features of novel coronavirus,influenza virus(A and B types),and adenovirus-induced pneumonia
目的 探讨常见几类病毒性肺炎的独特临床特征,明确这些特征对早期诊断与治疗有重要意义.方法 回顾性分析224例由新型冠状病毒、流感病毒(甲型和乙型)及腺病毒引起的肺炎患者的临床资料,利用Excel和SPSS软件进行数据统计与对比分析.结果 新型冠状病毒感染患者以高龄人群为主;腺病毒肺炎多见于中青年群体;甲型流感和腺病毒肺炎患者中,体温超过39℃的比例较高.乙型流感和腺病毒肺炎患者常伴有中枢神经系统症状,如头晕、头痛等.新型冠状病毒感染患者的淋巴细胞计数显著降低,碱性磷酸酶水平相对较低,但仍在正常范围内,而腺病毒肺炎患者入院时谷丙转氨酶升高.影像学检查显示,新型冠状病毒感染以小血管增粗、胸膜下分布为特征,而流感患者多存在渗出性炎症,胸膜下多出现楔形填充,腺病毒肺炎则表现为支气管壁增厚、磨玻璃影伴实变.不同病毒混合感染的病原体构成存在一定的差异.结论 不同类型病毒性肺炎在临床表现上具有一定的特异性.
Objective To explore the unique clinical characteristics of several common types of viral pneumonia and clarify that these characteristics are of great significance for early diagnosis and treatment.Methods A retrospective analysis was con-ducted on the clinical data of 224 patients with pneumonia clinically diagnosed as caused by COVID-19,influenza viruses(Type A and Type B),and adenovirus infections.Data statistics and comparative analyses were performed with Excel and SPSS soft-wares.Results The inpatients with COVID-19 were mainly elderly people.Adenovirus infection was more common among young and middle-aged people.Among patients with influenza A and adenovirus infection,the proportion of those with a body tempera-ture exceeding 39 ℃ was relatively high.Patients with influenza B and adenovirus infection often had symptoms of the central nervous system,such as dizziness and headache.The lymphocyte count of patients with COVID-19 was significantly decreased,and the level of alkaline phosphatase was relatively low,but still within the normal range.However,alanine aminotransferase was elevated when patients with adenovirus pneumonia were admitted to the hospital.Radiologically,COVID-19 was characterized by thickening of small blood vessels and subpleural distribution.Most patients with influenza had exudative inflammation,and wedge-shaped fillings often occurred under the pleura.Adenovirus pneumonia was manifested as thickening of the bronchial wall,ground-glass opacity accompanied by consolidation.In addition,there were certain differences in the composition of pathogens that caused co-infection with different viruses.Conclusion Different types of viral pneumonia have certain specificity in clinical manifestations.
