电针"天枢""上巨虚"调控NF-κB通路改善腹泻型肠易激综合征结肠低度炎症的机制研究OA
Study on the Mechanism of Electroacupuncture at Tianshu(ST25)and Shangjuxu(ST37)in Ameliorating Colonic Low-Grade Inflammation in Irritable Bowel Syndrome with Diarrhea by Modulating the NF-κB Pathway
目的 观察电针"天枢""上巨虚"对腹泻型肠易激综合征(IBS-D)模型大鼠长链非编码RNA NEAT1(lncRNA NEAT1)、肿瘤坏死因子受体超家族成员1B(TNFRSF1B)以及核因子-κB(NF-κB)的影响,分析电针治疗IBS-D的相关机制.方法 53只新生SD幼鼠随机分为正常组(12只)与造模组(41只).造模组采用母子分离联合醋酸灌肠及慢性束缚应激法建立IBS-D模型,最终筛选36只建模成功幼鼠随机分为模型组、抑制剂组及电针组,每组12只.抑制剂组腹腔注射NF-κB抑制剂PDTC(50 mg/kg),电针组大鼠电针刺激双侧"天枢""上巨虚"穴,每日1次,持续7 d.检测各组造模前、造模后和干预后稀便率与结直肠扩张痛阈值(AWR);干预后取各组结肠组织进行HE染色,ELISA法检测血清TNF-α、IL-1β、IL-8水平,qRT-PCR检测结肠组织NEAT1、TNFRSF1B、NF-κB p65 mRNA表达,Western blot检测结肠组织中TNFRSF1B、NF-κB p65蛋白表达,免疫荧光检测结肠组织中NF-κB p65、TNF-α、IL-1β、IL-8、TNFRSF1B阳性表达.结果 与正常组比较,模型组大鼠结直肠扩张痛阈值显著降低(P<0.01),稀便率明显升高(P<0.01),结肠黏膜呈现炎症浸润与组织结构紊乱,血清炎症因子TNF-α、IL-1β、IL-8含量升高(P<0.01),结肠组织中NEAT1、TNFRSF1B、NF-κB p65 mRNA表达量升高(P<0.01),TNFRSF1B、NF-κB p65蛋白表达水平升高(P<0.01),结肠组织中NF-κB p65、TNF-α、IL-1β、IL-8及TNFRSF1B阳性表达升高(P<0.01).与模型组比较,抑制剂组与电针组大鼠结直肠扩张痛阈值显著升高(P<0.01),稀便率明显降低(P<0.01),结肠组织形态学相对完整,炎症反应及炎性细胞浸润减轻,血清TNF-α、IL-1β、IL-8含量降低(P<0.01),结肠组织中NEAT1、TNFRSF1B、NF-κB p65 mRNA相对表达量降低(P<0.01),TNFRSF1B、NF-κB p65蛋白表达水平降低(P<0.01),结肠组织中NF-κB p65、TNF-α、IL-1β、IL-8及TNFRSF1B阳性表达降低(P<0.01).结论 电针刺激"天枢""上巨虚"穴可显著缓解IBS-D大鼠腹泻及内脏高敏症状,其治疗机制可能与下调NEAT1表达,抑制TNFRSF1B/NF-κB信号通路活化,从而减轻结肠低度炎症发挥治疗作用.
Objective To observe the effects of electroacupuncture at Tianshu(ST25)and Shangjuxu(ST37)on long non-coding RNA NEAT1(lncRNA NEAT1),tumour necrosis factor receptor superfamily member 1B(TNFRSF1B),and nuclear factor-κB(NF-κB)in a irritable bowel syndrome with diarrhea(IBS-D)rat model,thereby elucidating the specific mechanisms underlying electroacupuncture treatment for IBS-D.Methods 53 newborn SD rat pups were randomly assigned to control group(with 12 pups)and modelling group(with 41 pups).Pups in modelling group underwent maternal separation,colonic instillation of acetic acid,and chronic restraint stress to establish the IBS-D model.Ultimately,36 successfully modeled pups were selected and randomly assigned to the model group,inhibitor group,and electroacupuncture group,with 12 rats in each group.The inhibitor group received intraperitoneal injections of the NF-κB inhibitor PDTC(50 mg/kg).The electroacupuncture group underwent bilateral electroacupuncture stimulation at the Tianshu(ST25)and Shangjuxu(ST37),administered once daily for 7 consecutive days.Diarrhea rate and colorectal distension pain threshold(AWR score)at baseline,post-modeling,and post-intervention in each group were compared.After the intervention,colon tissue samples from each group were collected for hematoxylin and eosin(HE)staining.Serum levels of TNF-α,IL-1β,and IL-8 were measured using enzyme-linked immunosorbent assay(ELISA).The mRNA expression levels of NEAT1,TNFRSF1B,and NF-κB p65 in colon tissues were determined by qRT-PCR.Western blot was performed to assess the protein expression of TNFRSF1B and NF-κB p65 in colon tissues.Immunofluorescence staining was used to detect the positive expression of NF-κB p65,TNF-α,IL-1β,IL-8,and TNFRSF1B in colonic tissue.Results Compared with the control group,rats in the model group exhibited a significantly reduced AWR scores(P<0.01),and markedly increased diarrhea rate(P<0.01).The colonic mucosa exhibited inflammatory infiltration along with structural disorganization.Serum levels of the inflammatory cytokines TNF-α,IL-1β,and IL-8 increased(P<0.01).The mRNA expression of NEAT1,TNFRSF1B,and NF-κB p65 in colon tissues was increased(P<0.01),protein expression of NF-κB p65,TNF-α,IL-1β,IL-8,and TNFRSF1B increased(P<0.01),the positive expression of NF-κB p65、TNF-α、IL-1β、IL-8 and TNFRSF1B increased(P<0.01).Compared with the model group,rats in the inhibitor and electroacupuncture groups exhibited significantly elevated AWR scores(P<0.01)and markedly reduced diarrhea rates(P<0.01).Colonic tissue morphology remained relatively intact,with diminished inflammatory responses and reduced inflammatory cell infiltration.Serum levels of TNF-α,IL-1β,and IL-8 decreased(P<0.01),and the mRNA expression of NEAT1,TNFRSF1B,and NF-κB p65 in colon tissues decreased(P<0.01).The positive expression of NF-κB p65,TNF-α,IL-1β,IL-8,and TNFRSF1B in colon tissues decreased(P<0.01).Conclusion Electroacupuncture stimulation at Tianshu(ST25)and Shangjuxu(ST37)significantly alleviates diarrhoea and visceral hypersensitivity symptoms in IBS-D rats.Its therapeutic mechanism may involve suppressing lncRNA NEAT1 expression,blocking TNFRSF1B/NF-κB signaling pathway activation,thereby reducing the production of core inflammatory mediators and ameliorating colonic low-grade inflammation.
