肝纤维化是肝脏在病毒感染、酒精性损伤、非酒精性脂肪性肝病等多种致病因素长期作用下,肝细胞损伤后异常修复的代偿性反应.作为慢性肝脏疾病进展的核心病理环节,其特征为肝星状细胞活化增殖,伴随转化生长因子-β1、血小板衍生生长因子等关键细胞因子失衡,以及丝裂原活化蛋白激酶、Smad 等信号通路异常激活,最终导致胶原蛋白、蛋白多糖等细胞外基质过度沉积与降解失衡,持续进展可引发肝硬化、肝衰竭甚至肝细胞癌,已成为全球范围内严重威胁人类健康的公共卫生问题.本文系统综述了近年来肝纤维化发生发展过程中细胞层面、细胞因子调控、信号通路及细胞外基质代谢失衡的最新研究成果,重点梳理了各调控环节的交叉作用机制.笔者认为,肝纤维化的病理过程具有多细胞参与、多通路协同调控的复杂性,单一靶点干预效果有限.本综述通过整合现有研究证据,深入解析肝纤维化的核心调控网络,为进一步揭示疾病发病机制、开发多靶点联合治疗策略及寻找新型诊断标志物提供重要理论依据.

Hepatic fibrosis is a compensatory response of abnormal repair following hepatocellular injury,induced by long-term exposure to various pathogenic factors such as viral infection,alcoholic injury,and non-alcoholic fatty liver disease.As a core pathological link in the progression of chronic liver diseases,it is characterized by the activation and proliferation of hepatic stellate cells,accompanied by the imbalance of key cytokines including transforming growth factor-β1 and platelet-derived growth factor,as well as the abnormal activation of signaling pathways such as mitogen-activated protein kinase and Smad.Ultimately,this leads to the excessive deposition and degradation imbalance of extracellular matrix components such as collagen and proteoglycans.Uncontrolled progression can result in liver cirrhosis,liver failure,and even hepatocellular carcinoma,making it a major public health concern globally that seriously threatens human health.This article systematically reviews the latest research advances in the cellular mechanisms,cytokine regulation,signaling pathways,and ECM metabolic imbalance involved in the development and progression of hepatic fibrosis in recent years,with a focus on sorting out the crosstalk mechanisms among various regulatory links.The authors propose that the pathological process of hepatic fibrosis is characterized by the complexity of multi-cell involvement and multi-pathway synergistic regulation,and single-target intervention strategies yield limited therapeutic effects.By integrating existing research evidence,this review thoroughly analyzes the core regulatory network of hepatic fibrosis,providing an important theoretical basis for further elucidating the pathogenic mechanisms,developing multi-target combined therapeutic strategies,and identifying novel diagnostic biomarkers.
