医学人文教育中亲社会行为倾向的培养路径:基于反思性写作的整合研究OA
Cultivation paths of prosocial behavioral tendency in medical humanities education:an integrated study based on reflective writing
针对传统医学人文教育中"利他悖论"导致的职业倦怠困境,本文探索基于反思性写作的医学生亲社会行为倾向培养路径.采用混合研究方法,通过随机对照实验与质性文本分析,验证结构化反思性写作对亲社会行为倾向的干预效应.结果表明:实验组亲社会行为倾向得分(0.590±0.498)显著高于对照组(0.357±0.485,χ2=4.393,P=0.036),质性分析提炼出"亲社会认知""共情深化""困境应对"等五大核心主题,揭示反思性写作通过叙事重构、情境解构与共情映射三重机制,激活自我决定理论中的自主性、胜任感与归属感需求,推动医学生从技术操作导向转向"以患者为中心"的模式.研究初步证实反思性写作是缓解医学人文"知行分离"困境的可行尝试,为医学人文教育实践提供了理论与实践参考.
In response to the dilemma of professional burnout caused by the"altruism paradox"in traditional medical humanities education,this paper explored cultivation paths of prosocial behavioral tendency among medical students based on reflective writing.Adopting a mixed-methods research approach,this study verified the intervention effect of structured reflective writing on prosocial behavioral tendency through randomized controlled trials and qualitative textual analysis.The results demonstrated that the score of prosocial behavioral tendency in the experimental group(0.590±0.498)was significantly higher than that in the control group(0.357±0.485,χ²=4.393,P=0.036).Qualitative analysis extracted five core themes such as"prosocial cognition"and"empathy deepening"and"dilemma coping."It revealed that reflective writing,through three mechanisms of narrative reconstruction,situational deconstruction,and empathy mapping,activated the needs for autonomy,competence,and belongingness in self-determination theory,thereby driving medical students to shift from a technical operation-oriented model to a"patient-centered"model.This study preliminarily confirmed that reflective writing was a feasible attempt to alleviate the dilemma of"separation of knowledge and practice"in medical humanities,providing theoretical and practical references for the practice of medical humanities education.
